DESCRIPTION On August 17, 1975'at about 0840 hours0.00972 days <br />0.233 hours <br />0.00139 weeks <br />3.1962e-4 months <br /> the stack gas low OF OCCURRENCE: flow alara sounded in the control room. The shift forc=an investigated and found the particulato and charcoal filters in the "A" filter train required replaccuent due to normal loading.

Assuming that there were new filters already installed in "0" filter train, the foreman put "D" train in service. Ile then notified the radiation technician, who came down inmediately to replace the filters in "A" train. Af ter the filters had been changed the technician failed to notice that "B" train was still in service and therefore did not change the flow

~ path back thre:::h the filters in "A" train.

Th~is condition existed undetected until OS2S on August 20, 197S, when it was discovered ' during a routine filter change. Procedures had been prepared and approved by the Plant Operations Review Coccittee to cover any changes made in the stack sampling flow path or filter changes.

These had not been finally typed or distributed prior to this occurrence.
