' Occurrence Date August 29, 1975 Identification of Occurrence Violation of the Technical Specifications, paragraph 3.6. A.3. Failure of the stack gas sample system to monitor continuously stack releases while the reactor was in an unisolated condition. This event is considered to be an abnormal occurrence as defint:d in the Technical Specifications, paragraph 1.15 B.

Conditions Prior to Occurrence The plant was in a routine shutdown operation with the reactor mode switch in the refuel position and reactor coolant temperature less than 212 F.

Descript ion of Occurrence At 1833 on i, .aus t 29, 1975, a stack gas sample line low flow alarm was received in tm 'n t ro l room. The Shi ft Foreman was immediately noti fied and upon arriving c' the stack gas sample station, discovered stack gas sample pump "A", the pump that had been operating, not running. The thermal overload was reset and stack gas sample pump "B" started at 1843.

Stack gas sample pump "A" had just been returned to service earlier in the day after undergoing inspection and repair as a result of a similar occurrence on August 10, 1975.

'Ihe cause of this occurrence is believed to be a malfunctioning pump lubricator.

Analysis of Occurrence

%c safety significance of this event is considered minimal since at the time of the occurrence the reactor was shutdown and both the off gas system and the mechanical vacuum pump system were in an isolated condition. Additionally, a check of the reactor building ventilation exhaust monitors showed no radia-tion levels above background (%1.5 mr/hr.) .

Corrective Action The thermal overload protection (common to both pumps) was reset and the "B" stack gas sample pump put in service. The "A" pump was secured. Following this event, an inspection of the "A" pump revealed a broken diaphragm on the pump lubricator. De pump was reassembled after repairing the lubricator and tests will be conducted in order to determine the cause of the continuing fa ilures . Info 1mation accumulated from previous inspections of the stack gas sample pumps reveal that both pump motors are one (1) horsepower units, but the "A" pump's full load -- amperage is 14.4 amps versus 13.3 amps for the "B" pump. It was also noted that the "A" pump is constructed with a smaller frame si::e than the "il" pump, and because of this, it appears to be too small for its intended service. Gast Manufacturing Company, the supplier of both stack gas sample pumps, has been contacted in an effort to determine the present pumps design and operating capabilities and to investigate the possibility of replacing the "A" pump with an alternative pumping uni t .
