1. Report Number: A0-75-1 2a. Report Date: January 31, 1975 2b. Occurrence Date: December 6, 1974 (Classified as an A0 on January 20, 1975) 3 Facility: Palisades Plant, Covert, Michigan
4. Identification of Occurrence: lowable local leak rate exceeded on purge air supply valve (CV-1808),during test on December 6, 1974 and inadequacies in associated procedures.

5 Conditions Prior to Occurrence: Plant in cold shutdown.

6. Description of Occurrence: During a local leak rate test of Control Valve 1808, the operator measured a leak rate of 3 psi / minute around the packing and noted it at the bottom of the test data sheet. The leak was corrected by the operator and retested using the same data sheet. Upon completion of the test, a Test Engineer used the final (retest) results, which were acceptable, and did not observe the operator's note indicating the before-maintenance leakage.

On January 20, 1975, the Test Engineer received the test data and observed the before-maintenance note. A resultant calculation showed the leak rate to be unacceptable.

7 Designation of Apparent Cause of Occurrence: Lack of sufficient lubricant aggravated by unusual service condition.

8. Analysis of Occurrence: The Test Engineer, upon reviewing the tests on January 20, 1975, discovered the operator's note and recalculated the leak rate. The new calculation yielded a before-maintenance leak
rate of 283,421 cc/ minute which exceeded the leak rate limit. The after-maintenance data resulted in a leak rate of 509 cc/ minute after the packing was greased and reinstalled.

i l 9 Corrective Action: The following corrective action has or is being l taken:

a. The packing was properly 6reased and the leakage reduced to an acceptable level.
b. The Local Leak Rate Testing Procedure will be modified to identify "before-maintenance" and "after-maintenance" leak rate data.

d. A preventative maintenance program has been established to deter-mine an appropriate greasing schedule for valves susceptible to this type of problem.
