.o DESCRIPTION OF OCCURRENCE:

1-B engine (K-9204X) of I-A standby diecel - generator failed to startThe engine crankea prop during routine weekly survelliance testing.

(according to control room indication) but declutched after the exhaust gas temperature had not reached set-point in 30 seconds.

This information was lost, between the verbal communication and the log entries, and the surveillance test was run on schedule, resulting in an engine failure.

o ANALYSIS OF OCCURRENCE:

! The failure to have available to the operator a positive means of identifying l the status of the engines led to the abnormal occurrence. The failure of the

engine to start is not considered abnormal because of the uncertain condition

This is an abnormal occurrence per section 2.1 paragraph (g) of the Technical Specifications.

o CORRECTIVE ACTION:

A system of identifying to the operators, the status of all equipment (especially equipment not having a clearance issued, since a clearance board is aircady provided), is being developed and will be initiated as soon as practical.

FAILURE DATA /SIMILAR REPORTED OCCURRENCES:

Abnormal Occurrence No.12 is of a similar nature, though not exactly the same.
