Units were being run to check operability af ter a 6 months PM inspection.

After starting, loading, and running approximately 15 minutes, engine "B" of "A" generator tripped off on cooling water overtemperature.

A Plant Trouble Report was issued and the generator, with both A & H engines, run successfully at 700 RW for approxima'tely 30 minutes, on the follouing morning.

Prior to running the cooling water surge tanks were topped of f as per nornal procedure.

APPARENT CAUSE OF OCCURRENCE: X Design Unusual Service Cond.

. Including Environ.

Manufacture Component Failure Installation /Const. Other (specify)

Operator X Procedure After draining for maintenance, the water jackets sometimes require two or three topping off's between runs to climinate all air pockets and raise the coolant to its proper level.

This is norrally done under a " test" provision on the clearance card before the unit is returned to service by clearance surrender.

Eccause the tradesman van going off shift, he ' surrendered the cicatance, leaving the operator to conduct the final coolant fill and purging. On the maintenance man surrendering the clearance, the operator noted t he units as being returned to service. This vas an error on the part of the tradesnan.

ANALYSIS OP OCCURRENCF*

The water jackets were not properly fil. led , m per procedure. The operator was cartying out the proetdure uhen the unit tripp d. The diesel .hould still have been carried as under te.:t, not in neivice.
