IDENTIFICATION OF OCCURRENCE:
I ' During a routine 6 months Preventive Inspection of K-9206X ("D" engine j of "B" standby generator) a broken clutch disconnect mechanism was g discovered which rendered the declutch mechanism inoperable.
This'is identified as an Abnormal Occurrence, per Section 2.1, para (f) of the Technical Specifications.

When the declutching mechanism of K-9206X was examined during a routine 6 months Preventive Maintenance laspection, one end of the pin holding the declutch-collar to the clutch me danism was found to be sheared off. The pin fits through one end of p' art (5), of the bottom picture, page 1 of attachment.
Assembly is Part No. 6F6580, page 2 of attachment.
There was no damage to any.other components.
APPARENT CAUSE OF OCCURRENCE: Design Unusual Service Cond.
Including Environ.
Manufacture X Component Failure Installation /Const. ' Other (specify) e, Operator Procedure Through repeated operations the pin had failed.
The collar has a tendency to " chatter" on the shaft, giving a. load cycling effect on the pin. Being in a het location, lubrication tends to flow off, increasing the tendency of the collar to " chatter".
e ANAhYSIS OF OCCURRENCE:
With one end of the pin sheared it is doubtful that the disconnect mechanism would have worked successfully if called on to do so. The collar would probably have " skewed" and may have resulted in damage to the fork mechanism.
CORRECTIVE ACTION:
i All other enh nes have already' been
- inspected and have no pin failuren.
The pin will be replaced and the' frequency of lubrication increased'to keep'the collar from chattering.
