1. Description of Event:

With Unit I at 100% pow....,cor, a investigation revec.led that the amp readings for circuit 22A (Boron Inje::::tior:. Tank Inlet) Panels 1 & 2 were below the Acceptance Criteria Stipulated in ~he ?T. A power reduction and repair of the defective circuits was specifice~ i~ T.S.3.3.A.4 and is reportable in accordance with T.S.6.6.2.b(2)

2. Prob ab le Consequences & Status of Redundant Sys te::is:

A power reduction was initiated due to the failure of both redundant circuits All repairs and actions were within the Technical Specifications.The.heat tracing is intended to maintain. a te:ipe*rature abo:ve. th2.t :i.eeded _for flow -and flow *was maintained; therefore,tbe health and safety of the public were not affected.

3. Cause:

Since this tape 'b:?-d recently been replaced, the T!=-ason for failure: was an ..

excess-*amount* of tape a?plietl for the size of pipe heat requirements.

4. Irrrrnediate Corrective Action:

The irrrrnediate corrective action was to commence a power reduction and to replace the defective heat tracing tape. This infonnation will be provided to the task force §tud~.lLil.g trre heat.tracing systeI:c for further evaluatibn.

After replacing the heac tra::ing tape, operability of the affected Heat Tracing Circuit was verified and the unit returned to normal power operation.

5. Subsequent Corrective Action:

Since the subject circuit is continously monitored, no subsequent action is requir_ed.

6. Action Taken to Prevent Recurrence:

No additional actions were deemed necessary.

7. Generic Implications:

A task force has been estc.:1Eshed to study the total spectrum of the heat tracing system in an atceqt to discern present p:-oblems and recommend solutions which will eli~i~ate sporadic failures experienced.
