ACCEPTANCE TESTING OF CONCRETE MATERIALS The following status report is offered as partial fulfillment of the require-ments of 10CFR 50 55(e)(3).

Description of the Deficiency Delta Testing, Contractor to Stone & Webster for work defined in specifi-cation 210.360, " Concrete Testing Services", subcontracted portions of the work to Southwestern Laboratories. A Quality Assurance (QA) audit of South-westera Laboratories was conducted by S&W on Septem'oer 25 and 26,1979, to determine if activities were performed in accordance with specified require-ments. The audit revealed that QA requirements had not been satisfactorily implemented by Southwestern Laboratories, particularly in the area of cali-bration, reporting, and existing shop practices. It was concluded that assurance and confidence could not be provided that work performed and materials tested satisfied specification requirements.

The primary cause of the Southwestern Laboratoi>cs incident was tl.e failure of Southwestern's Dallas and Fort Worth facilities to comply v.th their company's Quality Assurance Manual. A secondary cause was the failure of Delta Testing and Inspection, Inc. , to adequately evaluate Southwes tern Laboratories' implementation of their own QA Manual. Tests performed cy Southwestern Laboratories consisted of acceptance testing of Category I concrete materials -specifically cement, fine and coarse aggregates, uad the mix water.

An analysis of the safety implications and a statement of corrective actions as required by 10CFR 50.55(e)(3) vill be provided to you in a su= mary report by December 30, 1979 8002220dTo/
