-DEFECTS/NONCOMPLIANCE NOTIFICATIONS UNIT SCRAM CODE RX CRIT INIT PWR INIT RX MODE CURR PWR CURR RX MODE N 100 POWER OPERATION 100 POWER OPERATION EVENT TEXT THE FOLLOWING INFORMATION IS TAKEN FROM THE TRANSMITTED PART 21 REPORT.'IT HAS BEEN DETERMINED THAT CERTAIN LIMITORQUE BRAND MOTOR-OPERATORS AT WNP-2 HAD UPPER HOUSING COVER CAP SCREWS WHICH, HAD THE MOTOR-OPERATORS BEEN SET AT 1004 OF THE MANUFACTURER RATED THRUST, COULD HAVE FAILED IN-SERVICE.
THIS CONDITION WAS CAUSED BY THE INSTALLATION OF GRADE 1 OR 2 CAP SCREWS INSTEAD OF GRADE 5 OR 8.ALTHOUGH GRADE 1 OR 2 CAP SCREWS WERE FOUND IN SEVERAL MODELS OF LIMITORQUE MOTOR-OPERATORS, FAILURE OF THE CAP SCREWS IN-SERVICE DUE TO THE RATED THRUST LOADS WOULD ONLY BE POSTULATED FOR SMB-000 MODEL.'~pefppf>
