ATTACHMEMT 1 SURRY POWER DOCKET NO: REPORT NO: EVENT DATE: STATION, UNIT 2 50-281 80-031/031-0 10-15-80 TITLE OF REPORT: PIPE SUPPORT IMPROPERLY INSTALLED

1. DESCRIPTION OF EVENT: e With Unit 2 operating at 100% power, the final as-built review of supports stalled as -a result of IE Bulletin 79-14, showed an overstressed pipe -support in the alternate charging system (4-CH-389, FC-1298A).

This event is reportable as per IE Bulletin No. 79-14-Supplement

2. 2. PROBABLE CONSEQUENCES AND.STATUS OF REDUNDANT SYSTEMS: The overstressed support could.result in a pipe support failure, and pipe overstressed condition.

Since this sub system (alternate charging system) was isolated upon notification

  • of a possible overstress condition, the health safety of the public were not affected.
3. CAUSE OF EVENT: The pipe support was not ~nstalled as per.design requirements.
4. IMMEDIATE CORRECTIVE ACTION: Isolated the alternate charging header by closing MOV':'"2287A, B, and C. 5. SUBSEQUENT CORRECTIVE ACTION: Corrective action was taken to modify the supports in accordance with liminary design drawings . .These modifications were completed within the same day as the notification of an overstressed condition.
  • The alternate charging header was returned to service after completing the modification.
6. ACTION TAKEN . TO PREVENT RECURRENCE:

None required.
