1. Description of Event: Construction personnel were installing electrical cables as a part of a plant improvement project. A fire seal in the fire barrier between the Auxiliary building and Unit One cable tunnel had been removed to facilitate the pulling of cables through the conduit. Due to administrative oversight, the fire barrier was not resealed prior to workers departing the area. This event is contrary to Technical Specification 3.21.G~ In another instance, after cables had been installed in penetrations of a fire barrier which is required to perform a pressure sealing function, the three penetrations involved were resealed but a local leakage test to verify operability was not conducted immediately.

This is contrary to Tech-. nical Specification 4.18.H. 2. Probable Consequences/Status of Redundant Systems: The penetration of the fire barrier was unsealed for approximately 11 hours1.273148e-4 days <br />0.00306 hours <br />1.818783e-5 weeks <br />4.1855e-6 months <br />. In the event of a fire in the immediate vicinity, there was a remote possibility the fire could have traveled to the other side of the wall be-' tween the Auxiliary building and Unit One cable tunnel. The lack of a local leak test in the other wall prevented positive assurance of the pressure boundary.

3. Cause: The cause of this event was a lack of adequate administrative control over construction activities involving fire seals. Appropriate corrective actions have been initiated.
4. Immediate Corrective Actions: Upon discovery of the unsealed fire barrier, a fire watch was posted until the fire seal was reinstalled.

A leak test was successfully conducted in the other case. 5. Scheduled Corrective Actions: There is no corrective action required.

The personnel involved have been reinstructed on the procedure ments. Additionally, a working log has been initiated to be time-dated and signed by the foreman which will insure continuity of sealing requirements are adhered to. Also, a VEPCO supervisor will be notified prior to and upon pletion of all work on fire seals. He will insure compliance with pertinent instructions.
