Conditions Prior to Occurrence: 100% Full Power Description of Occurrence:

At 0700 on April 28, 1978, PT/1/A/0251/01 (Low Pressure Service Water Pump Performance Test) was commenced. The test required the throttling of 2LPSW-251, among other valve alignments, to establish various flows. The test was completed at 1130. At 2355, personnel discovered that three valves, including 2LPSW-251, had not been returned to their r.ormal position following the tests.

The closure of 2LPSW-251 i; moved the 2A Low Pressure Injection (LPI) Cooler f rom service. The valves were returned to normal at 2400.

Cause of Occurrence:

The cause of the incident was procedural deficiency. The procedures for PT/1/A/0251/01 failed to indicate a step which returned the system to its normal condition. ,

Analysis of Occurrence:

The valve throttling (approximately 80% closed) ef fectively removed the 2A LPI Cooler f rom service. This caused one train of the LPI system to be inoperable for a period of approximately 14 hours1.62037e-4 days <br />0.00389 hours <br />2.314815e-5 weeks <br />5.327e-6 months <br />. The Oconee Technical Specifications allows one train to be inoperable for periods not to exceed 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> for test-ing and repairs (TS 3.3.5). The capacity of one LPI train is suf ficient to supply decay heat removal capability. Thus, the health and safety of the general public were not endangered.

Corrective Actions:

The procedure in question, PT/1/A/025/01, has been revised to provide a step to return the system to normal conditions. Also, other procedures relating to ES equipnent are being reviewed to assure-that no similar deficiencies occur elsewhere.
