On September 24, 1982 the NRC approved a partial release of Stop Work Order 19, Containment Structural Steel, to allow reinspection of fixed connections in five quadrants of the containment. This partial release was labeled 2A-1 and was completed on October 22, 1982; the complete report is attached.
Partial release 2A-1 consisted of inspection of 449 fixed connections in the five quadrants of containment where drawings were inadvertently destroyed. Inspection of the 9 drill-and-tap connections tangential to box girders in release 2A-1 identified 4 connections with oversize holes that had not previously been identified as non-conforming. These four connections will be re-paired by welding. Consequently the remaining 28 of this type of connection already installed will be inspected and evaluated for potential corrective action in phase 2A-2. However, since the reinspection in release 2A-1 was performed as a result of lost documentation, it is not possible to determine if the 4 non-conforming connections had been previously inspected.                With this exception the quality of the fixed connection installation has                                  -
been proven to be in conformance with applicable requirements.
A potential 10CFR50.55(e) has been filed with the NRC concerning beam to box girder drill-and-tap connections. While these connections were found to be in conformance with applicable drawings, the adequancy of existing inspection criteria to assure sufficient thread engagement exists for the development of bolt strength is being questioned. This will be evaluated and resolved.
