1. Description of Event:

An inspection of the Unit 2 containment, following a Type A Leak Rate Test, disclosed that eight (8) 1/4 inch holes had been drilled through the containment liner~ This event is ]:'.eportable in accordance with Technical Specification 6.6.2(a).5.

2. Probable Consequence and Status of Redundant Systems:

These holes and other minor construction deficiencies contributed to an unsatisfactory Type A Leak Rate Test. Since Unit No. 2 is in a shutdown and defueled condition the health and safety of the general public were not affected.

3. Cause:

During the implementation of Design Change 78-48 to install RTD's in the containment, holes were drilled through the liner as a result of improper interpretation of the implementation procedure.

4. Innnediate Corrective Actions:

The problem was referred to the Architect Engineer to generate a procedure to repair the holes.

5. Subsequent Corrective Actions:

The procedure has been received by the station and repair to all the holes will be completed prior .to recommencement of the Type A Test. Repair of the other minor construction deficiencies impacting on the Type A Test will also be completed prior to conducting the Test.

6. Action Taken to Prevent Recurrence:

Construction personnel have been reinstructed on the intent of the working procedures to insure they understand and do not perform any work not specifically covered in the procedure.
