HARTSVILLE AND PHIPPS BEND NUCLEAR PLANTS - REPORTABLE DEFICIENCY -
FAILURE BY ITT GRINNELL TO USE PROPER TESTING METHODS - HTRD 50-518/81-08, -519, -520, -521/81 PBRD-50-553/81-10 The subject deficiency was initially reported to NRC-0IE, Region II, Inspector Pete Taylor on March f+, 1981 as NCR-7                The first, second, and third interim reports were submitted on April 3, May 19, and August 11, 1981, respectively. In compliance with paragraph 50.55(e) of 10 CFR Part 50, this letter constitutes the fourth interim report on the subject deficiency. TVA has returned all nonconforming subassemblies to ITT Grinnell Industrial Piping shop in Kernorsville, North Carolina. ITT Grinnell has agreed to perform magnetic particle or liquid penetrant inspection,in accordance with NB-2551(b) (ASME Section III, Winter Addenda '.971) on all accessible external and internal surfaces of the fittings in question. ITT Grinnell has proposed to perform NDE's on internal surfaces of in-stock fittings of the same heat number. This method would alleviate the need for removal of selected fittings from spool pieces by sectioning for NDE's of unaccessible inside surfaces.
However, at the present time, it has not been determined whether the results of NDE's of in-stock fittings of the same heat numbers can be used to satisfy TVA's requirement for statistical sampling of unaccessible fittings that are welded in the subject subassemblies. TVA anticipates transmitting the final. report on or before June 22, 1982.
The 10 CFR Part 21 implications of this deficiency '4111 be determined by the testing and evaluation. If you have any questions, please call Jim Domer at FTS 858-2725.
