FACILITY OPERATING LICENSE NPF-30 OWNER'S ACTIVITY REPORTS (OAR-1 FORMS) FOR REFUEL 18 Please find enclosed the Owner's Activity Reports (OAR-I forms) for Refueli8 activities at Callaway Plant. The reports are submitted pursuant to the provisions of ASME Boiler and Pressure Vessel Code Case N-532-4, "Alternative Requirements to Repair and Replacement Documentation Requirements and Inservice Summary Report Preparation and Submission."

The OAR-I forms enclosed are associated with the following Refuel I8 activities at Callaway Plant:

I. lnservice Inspection and Repair/Replacement activities, and

2. Containment Inservice Inspection pursuant to ASME Section XI, subsections IWE and IWL.

In accordance with Code Case N-532-4, each attached OAR-I form includes a listing of items with flaws or relevant conditions that required evaluation for continued service (Table I) and an abstract of repair/replacement activities required for continued service (Table 2).

The evaluation determined that the blistered paint was not a result of corrosion or other distress to the liner base metal and the condition was therefore acceptable.

E-A, E1.11 Containment Normal CAR 201109796 identified corrosion on the 'A' Sump Liner containment sump liner plate that did not meet acceptance criteria. The evaluation determined that there was no structural degradation due to the corrosion and the condition was acceptable.
