RENEWED FACILITY OPERATING LICENSE NPF-30 OWNER'S ACTIVITY REPORTS (OAR-1 FORMS) FOR CYCLE/REFUEL 26

Enclosed are the Owner's Activity Reports (OAR -1 forms) for Cycle 26 and Refuel 26 activities at Callaway Plant. The reports are submitted pursuant to the provisions of the applicable revisions of ASME Boiler and Pressure Vessel Code Case N-532-5, "Alternative Requirements to Repair and Replacement Documentation Requirements and Inservice Summary Report Preparation and Submission," and Code Case N -892, "Alternative Requirement for Form OAR -1, Owner's Activity Report, Completion Time Section XI, Division 1."

The OAR-1 forms enclosed are associated with the following Cycle 26 and Refuel 26 activities at Callaway Plant:

1. Inservice Inspection (ISI) and Repair/Replacement activities for the third period of the fourth ISI Program interval, and

2. Containment Inservice Inspection pursuant to ASME Section XI, subsection IWE and IWL for the third period of the third IWE and IWL Program interval.

Each enclosed OAR-1 form includes a listing of items with identified flaws or relevant conditions that required evaluation for continued service (Table 1) and an abstract of repair/replacement activities required for continued service (Table 2).

In accordance with Revision 4 of the Fourth Interval ISI Program Plan, the OAR-1 form for the ISI and Repair/Replacement activities is submitted in accordance with Code Case N-532-5. Similarly, in accordance with Revision 4.1 of the IWE Program Plan, and Revision 4 of the IWL Program Plan, the OAR-1 form for the Containment Inservice Inspection activities is submitted in accordance with Code Case N-532-5.
