APPEllDIX A NOTICE OF VIOLATION Power Authority of the State of New York Docket No. 50-286 Indian Point Unit 3 License No. DPR-64 As a result of the inspection conducted on May 16-June 15,1982, and in accordance with the NRC Enforcement Policy (10 CFR, Part 2, Appendix C) 47 FR 9387 (March 9,1982), the following violation was identified:
Technical Specification 6.8.1 requires that written procedures be implemented covering the applicable procedures recorsnended in Appendix "A" of Regulatory Guide 1.33, November,1972.
Regulatory Guide 1.33, November, 1972, G.2, Solid Waste System, lists baling rachine operation as a typical safety related activity that should be covered by a written procedure.
SOP-IDS-5, " Baling liachine Operation," Step 4.3 states in part,
....see that the drua is centered under plate, close the shroud door.
Contrary to the above, on l'ay 27, 1982, solid radioactive waste operations were conducted, including the use of a baling machine, without the use of a procedure, and during operation, the shroud door was not closed as required.
This is a Severity Level 5 Violation, (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Power Authority of the State of New York, is hereby required to submit to this office within thirty days of the date of the Notice, a written statement or explanation in reply, inclu-ding (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending your response time.
