Special Reporting Requirement for Inoperable Seismic Monitoring at Limerick Generating Station

Reference:

Technical Specification 3.3.7.2 and 6.9.2

Dear Mr. Russell:

This special report is being submitted pursuant to the q requirements of Limerick _ Technical Specifications 3.3.7.2 and 1 6.9.2 which state: i l

-SEISMIC' MONITORING INSTRUMENTATION:

l 3.3.7.2 The seismic monitoring instrumentation shown in Table 1 3.3.7.2-l* shall be OPERABLE.

APPLICABILITY: At all times.

ACTION:

a. With one or more of the above required seismic monitoring instruments inoperable for more than 30 days, prepare and submit a Special Report to the Commission pursuant to

, Page 2 Specification 6.9.2 within the next 10 days outlining the cause of the malfunction and the plans for restoring the instrument (s)'to OPERABLE status,

b. The provisions of Specifications 3.03 and 3.04 are not applicable.

DESCRIPTION OF EVENT: .

1 On May 18,-1987 with the unit shutdown, Triarial Peak Recording Accelerograph KR-VA-151 was removed from.the reactor pressure vessel head in preparation for refueling. The Triarial Peak l Recording Accelerograph is a passive device that mechanically .

records the accelerations of a seismic event. This instrument is i listed in Technical Specification Table 3.3.7.2-1 and is required i to be operable as stated in Technical Specification 3.3.7.2 above.

CONSEQUENCES OF THE EVENT:

The consequences of this condition are minimal. The control room will continue to receive notification of a seismic event because  !

the control room annunciators receive signals from components of the seismic monitoring system which are operable. Limerick has three Triaxial Peak Recording Accelerographs of which two are j still operable.  !

CAUSE OF EVENT:

The cause for the inoperability of XR-VA-151 was the need to i remove this instrument in support of removing the vessel head for refueling. >

CORRECTIVE ACTIONS:-  !

XR-VA-ISl will be reinstalled after completion of refueling l activities and replacement of the vessel head. Surveillance Test i ST-2-036-402-0 will be performed to demonstrate operability of 1 the instrument.
