Limerick Generating Station, Unit 1 Special Report for Inoperability of the Loose-Part Detection System for More Than 30 Days This Special Report is being submitted pursuant to the requirements of Limerick Generating Station (LGS), Unit 1 Technical Specifications (TS) Section 6.9.2 as required by TS Section 3.3.7.10,
" Loose-Part Detection System."
TS Section 3.3.7.10 requires that with one or more of the required Loose-Part Detection System (LPDS) channels inoperable for more than 30 days, a Special Report must be prepared and submitted within 10 days outlining the cause of the malfunction and the plans for restoring the channel (s) to operable status.
On April 22, 1994, with Unit 1 operating at 100% power, Instrumentation and Controls (I&C) personnel performed Surveillance Test (ST) procedure, ST-2-036-640-1, " Loose Parts Monitoring System Channel Functional Test."
During the test, the tape recorder would not advance tape in the play mode.
The tape recorder is used to record the acoustic signal from the individual LPDS channels.
The tape recorder automatically starts recording when a preset acoustic level is exceeded on any one channel.
Without the recording, the acoustic information cannot be analyzed.
The I&C personnel contacted the Main Control Room personnel and all eight LPDS channels were declared inoperable on April 22, 1994 at 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br />.

May 31, 1994 Page Two The tape recorder could not be repaired on site and the tape recorder was returned to the vendor for repairs.
The vendor replaced the capstan be.lt pinch roller and the capstan motor which had failed due to normal wear.
The tape recorder was returned to the site and following satisfactory 9 ting, the recorder was declared operable.
Operations persorinel then declared six of the eight LPDS channels operable on May 26, 1994, at 2145 hours0.0248 days <br />0.596 hours <br />0.00355 weeks <br />8.161725e-4 months <br />.
Two of the LPDS channels remained inoperable as previously stated in a Special Report submitted on April 19, 1994.
