1.0 ' NTRODUCTION

'This document presents the expanded investigation program of the Bostrom-Bergen/Meddco Metals' (BB/MM) ollegations prepared by Pacific Gas and Electric (PGondE). This program is consistent with the commitment to perform reinspection of a logical representatiwe sample of BB/MM supplied materials made to the U. S. Nuclear Regulatory Commission (NRC) Region V Staff on Thursday, February 16, 1984, and will "

be integrated into the existing investigation initiated in December 1983.

The objective of this program is to provide odditienal confirmation that BB/MM-supplied items meet requisite quality standards (refer to g*ograph 2.5) by performing field reinspections. A further objective of the program is to provide this information through a well thought-out plan which leads to o logical representative sampling program.

2.0 SCOPE This scope consists of the following eight tasks which will ossure that a satisfactory reinspection program is performed.

2.I The first task is to finalize the identification of the scope of materials supplied to the Diablo Canyon Power Plant (DCPP) by BB/MM. This requires review of BB/MM-supplied structural items for all periods of interest and will be accomplished at the DCPP site. The information that will be gathered in this task will be based on the lists of BB/MM material used at DCPP as shown in Attochrnents A and B. The lists include all major pieces shown on the BB/MM shop drawings.

2.2 The second task is to define key chorocteristics (those chorocteristics which determine the manner in which BB/MM item grouping occurs) related to the different types of items supplied by BB/MM. These will include:

l 2.3 , The results shown in Attachments A and B, which represent all major BB/MM items supplied to DCPP, will be onelyzed and grouped into generic item categories. This grouping will ossure that the items are identified in terms of the l

components, materials and. time frames of supply. In this way, a basis for o logical representative sompte selection will ofready be established.

2.4 Upon completion of the categorization, logical sompie groups for reinspection will

. be determined to assure o representative reinspection across the population os BB/MM-supplied items and will provide assurance that PCondE has on indication of the quality of the BB/MIA items.

Emphasis in selecting items for, inspection will be placed on (1) confirming that BB/MM supplied special steels were used at locations where they were required, and (2), confirming that special fabrication techniques performed by BB/MM (e.g.,

welding) were odequately performed. Selection of individual items for inspection

- will be made to cover os brood a time frame os possible. In summary, the total l

l quantity of items to be inspected is large enough to encompass material used in all safety-related gplications over the periods of interest for BB/MM material supplied to Diablo Canyon.

h Several BB/MM-supplied materials have been reinspected in great detail. These are identified in Attochment C and will be incorporated into the program.

2.5 Field inspections will be performed to provide assurance that the existing materials meet specified requirements. Inspection of sompted items will cover two genc rol creas:

1. Conformance of BB/MM materials to design and specification requirements.
2. Conformance of BB/MM fabrication to acceptable practices and to design drawing requirements.

2.5.1 Inspections of BB/MM material will include:

! o. Hordness testing to confirm the tensile strength of the base material. A form similar to Attochment D will be used for this inspection.

b. Alloy analysis to confirm the content of specified chemical elements in the base material. A form similar to Attachment E will be used for this inspection.

2.5.2 Inspections of BB/MM fabrication will include:

L o. Visual examination, per AWS criterio, for weld quality and dimensional verification of weld sizes and base metal thicknesses. A

form similar to Attachment F will be used for this inspection.

b. Measurement of critical dimensions and visual examination of overall conformance to design drawing requirement.

i 2.6 A final report will be prepared upon completion of the BB/MM materials reinspection program, documenting each of the tasks performed and fully explaining BB/MM grouping and how the samples were selected. The final results and conclesions will be fully documented and submitted to the NRC.

NUCLEAR PLANT QUALITY ALLEGATIONS MADE TO LOCAL TV STATION A former welding inspector at Bostrom Bergen Metal Products Company of Oakland, California, has contacted a San Francisco television station, KGO-TV (ABC), with allegations that he falsified various inspection documentation on fabricat;nd metal products sold to Diablo Canyon, South Texas, Comanche Peak, and an unidentified WPPSS project. The television station has contacted Region V and Bostrom Bergen concerning the allegations but has not yet aired a story.

The TV reporter contacted the Region V Public Affairs Officer about the allegations on the afternoon of December 6. The reporter put the alleger in contact with the Director of the Region V Office of Investigations.

On December 7, 1983, an executive of Bostrom Bergen contacted the Region V duty officer. He said that Bostrom Bergen had been confronted with these allegations by a TV reporter several days earlier. He said the Company has conducted an investigation'which has largely refuted the allegations.

The Region V inspection team at Diablo Canyon looking into other allegations has been made aware of the new charges. The Region IV Vendor Inspector Branch has been notified.

NUCLEAR PIANT QUALITY ALLEGATIONS MADE TO LOCAL TV STATION A former welding inspector at Bostrom Bergen Metal Products Company of Oakland, California, has contacted a San Francisco television station, KGO-TV (ABC), with allegations that he falsified various inspection documentation on f abricated metal products sold to Diablo Canyon, South Texas, Comanche Peak, and an unidentified WPPSS project. The television station has contacted Region V and Bostrom Bergen concecning the allegations but has not yet aired a story. The TV reporter contacted the Region V Public Affairs Officer about the allegations on the afternoon of December 6. .The reporter put the alleger in contact with the Director of the Region V Office of Investigations. On December 7, 1983, an executive of Bostrom Bergen contacted the Region V duty officer. He said that Bostrom Bergen had been confronted with these allegations by a TV reporter several days earlier. He said the Company has conducted an investigation which has largely refuted the allegations. The Region V inspection tean at Diablo Canyon looking into other allegations has been made aware of the new charges. The Region IV Vendor Inspector Branch has been notified. In addition, the quality of the hardware fabricated may be questionable based on allegations that welder qualification records were falsified, inspectors lacked QA training, and material certifications may have been falsified. Implied Significance to Design, Construction or Operation Failure of structural component supports and pipe rupture restraints to perform their intended function is a potential safety problem. Staff Position The allegations were directed at alleged violations in the quality control practices utilized by Bostroin-Bergen Metal Products (BBMP) at their Oakland, California fabricatio'n plant. If these allegations are proven correct then the quality of the BBMP hardware installed at Diablo Canyon'would be questionable. Therefore, the final report on i this allegation will address reactive inspections conducted at both the fabricator's shop and at Diablo Canyon.

a 9 4 A. Inspection at Diablo Canyon

1. Visual Inspection of Installed Hardware o
  • The licensee, at the inspector's request, located and ,

identified BBMP hardware installed in safety-related systems. The in.spector visually examined these items for evidence of material damage and weld quality. Weld inspection,was limited because all the items had received a protective coating of paint. No material damage was identified. Weld location, length, size and shape were within construition specifications; weld reinforcement, appearance and. type (butt or fillet) were as required. Evidence of surface defects such as cracks, porosity, or slag were not inspectable due to surface paint. The areas inspected during this walkdown and the type of hardware inspected are as follows: , (a) Turbine Building - CCW heat exchanger barrier, rupture restraints for CCW heat exchanger barrier, leveling plates for the 4160 volt vital Buss H in the switch gear room, and diesel' generator 1-1 embed plates. (b) Auxiliary Building - AFW 1-1 pump missile shield, SI pump 1-1 and 1-2 embed slaps, containment spray emeeds, RHR pump 1-2 anchor bolts, and BIT anchor bolts.

4 (c) Unit 1 Containment - Main Steam Generator FW line rupture restraint no. 1047/10RT, enibed plates for fan coolers, embeds for pressurizer wall and platforms, and annulus platform beams.

Attributes examined were material, welding process, filler metal, welding current, electrode size,. preheat and interpass temperiture. Assessment of Safety Significance At the time SSER No. 21 was written BISCO was not a contractor or subcontractor,nor had they ever been, at Diablo Canyon. However, i' from December 10, 1983 through February 1, 1984 BISCO performed work \. [. on 179 safety-related pressure seals. The engineering significance of the' allegation has been reviewed and l ! . no significant safety problems have been identified. l l t

Staff Position The staff concludes that no safety concern exists at Diablo Canyon. This conclusion is based on the inspector's review.of BISCO's quality assurance program and quality control procedures implemented during , BISCO's work performed at Diablo Canyon. The staff's inquiry into the extent of PG&E QA/QC surve.illance of in process work revealed that no formal audit or surveillance had been performed. The licensee's explanation-was that, BISCO's work scope was so limited, i.e. for a short period of time that a formal audit was not scheduled. However, the licensee is in process of performing a quality control inspection of 5% of BISCO's work to verify proper installation of pressure seals. The staff's conclusion is based on interviews with licensee personnel and review of the following BISCO documents:

       . Quality Assurance Program and Procedures The inspector. reviewed the BISCO quality assurance program as described in the BISCO Quality Assurance Manual (BQAM), Revision 9, dated 2-5-82. A letter from the President of BISCO is enclos?d committing BISCO to pdrform all work in accordance with the BQAM which implements the requirements of 10 CFR 50 Appendix B and ANSI N 45.2. The BQAM co'nsists of 18 sections each of which defines the requirements or describes the methods to assure compliance with the corresponding 18 criterion of Appendix B to 10 CFR 50. The BQAM was reviewed and approved by the licensee Quality Assurance Department prior to BISCO performing work.

allegations. These concerns can basically break down into three chronological groups:

1. 1981 to 1983 -'The initial allegation involving a former inspector who claims to have documented inspections he did not perfo.rm.
2. 1979 to 1980 - Allegations regarding maintenance of welder qualifications, lack of QA training, shipment of discrepant' material to nuclear projects, questionable subsuppliers, and working conditions.
3. 1977 to 1979 - Allegations regarding a Bostroe-Bergen subsidiary (Meddco Metals) relating to material traceability, removal of
                                        . reject tags, and use of foreign produced steel.

The NRC staff response to the allegations includes a combined effort by the Office of Investigations, the Licensee Contractor and Vendor Inspection Program Branch of the Office of Inspection and Enforcement, and Region V. The staff position has been both one of monitoring how the licensee is conducting his investigation for the Diablo Canyon Project and independently reviewing

               - the issues for generic significance (the company has provided products to multiple nuclear reactor projects)., The staff's approach to these concerns includes:                                 .
1. monitoring licensee actions and independently conducting interviews with a number of current and former employees of the company and ,its subsidiaries; 1

2. examining of the quality program employed by the company currently, and in the past; 3. reviewing contractor and licensee records related to the work in question; 4. and physically inspecting material installed at Diablo Canyon. The staff has addressed and closed the original allegation. A review of pertinent records established.that the former inspector (who claims to have documented inspections he did not perform) is credited with performing 650 inspections while he was employed at Bostrom-Bergen. ' Fifteen of the 650 inspections involve safety-related material. ' These fifteen items were found to be supplied to Diablo Canycn Unit 2 and involve " stock" material '(i e. . raw material items which do not involve welding). As of this writing the staff has inspected 14 of the 15 items and found them to conform with requireaents . The s'taff is following up on the last ites (plate washers). The licensee has l selected a 10% sample of the other (non-safety related) inspections related to j the inspector and performed a reinspection (involving 940 welds) . Seven of the 940 reinspected welds were found to have deviations from requirements , these are being properly addressed. Based upon the low defect rate the licensee has concluded that the structures and components installed at Diablo Canyon have-not been adversely impacted by the former inspector's alleged performance. The staff-concurs with ,this conclusion based upon a := view of licensee actions and independent inspection of the fifteen safety-ralated items .

The staff has completed a substantial amount of review on the second and third groups of allegations, and to date has not identified problems of safety . significance, the reviews, however, are continuing (e.g. the staff has not completed their review of the Meddco Metals operations). These allegations are mainly general in nature, lacking in specific examples thus requiring extensive interviewing and document reviews. In a parallel effort the licensee has initiated an inspection of installed hardware to allow a direct assessment of material adequacy, separate from the management and programmatic concerns related to the vendor. Items that are being reinspected were selected by reviewing all shop drawings and selected purchase orders involving the vendor's material shipped to the jobsite since 1969 and includes samples of each material type supplied to Diablo Canyon with particular attention to items which are difficult to fabricate or involve special materials. i 90% of the sampling has been completed and the licensee reports that the following trends and results are apparent: r

c) Visual weld inspections are indicating that vendor welding meets design requirements. d) Records from the NDE documentation research show that full penetration welds by the vendor are satisfactory. - 4 In addition to the licensee's reinspection the staff has independently inspected a small sample (14 types of components) of installed safety related hardware to obtain first hand evidence of product quality. The components l were visually inspected for material damage, weld location, length, size, i ~ shape, reinforcement, appearance and type. The staff did not identify any discrepant material. Records related to this material were reviewed and appeared to be in order. Staff Position Investigations and reviews have been completed on the initial and most alarming allegations. This item is resolved. The reviews are continuing on the other two sets, but, to date significant safety problems have not be:n identified. Based upon staff findings to date and the acceptable results of reinspection of installed hardware it is the' staff's opinion that this issue j no longer requires full resolution prior to a reactor criticality decision. The staff estimates that their investigations will be completed by aid May ( 1984. Action Required

SUMMARY

i PGandE has recently completed a review of the materials and fabrications supplied by Bostrom-Bergen/Meddco Metals (BB/MM) to the Diablo Canyon Power i Plant (Diablo Canyon). This review was accomplished by performance of hardware reinspections and by reviewing the results of previous reinspections of BB/MM-supplied products. The following report describes the method used to select a representative sampling of BB/MM materials for reinspection. This report also describes the methods used to reinspect the selected materials and fabrications and provides the results of the current and previous reinspections. Evaluation of the reinspection results has-shown that acceptable materials and fabrications were provided to Diablo Canyon. Thus, it is concluded that the BB/MM quality assurance program did, in fact, provide for materials and fabrications that meet design requirements. l

e s FINAL REPORT ON BOSTROM-BERGEN/MEDDC0 METAL MATERIALS SAMPLING PROGRAM l INTRODUCTION As a result of allegations regarding the quality assurance practices of Bostrom-Bergen/Meddco Metals, PGandE has performed a reinspection of representative materials supplied by BB/MM to the Diablo Canyon Power Plant.. 8 In PGandE's Letter No. DCL-84-081, dated February 29, 1984,'an interim report on the sampling of the BB/MM materials was provided. This final report concludes PGandE's sampling of the materials supplied by BB/MM. . The objective of the sampling program was to provide additional confiderce, through performance of hardware reinspections, that the quality assurance program in place at BB/MM provided for materials that meet design requirements. To complete the tasks outlined in the interim report, the scope of the sampling program has been finalized and the reinspection of the items selected for review has been completed. The results and conclusions reached as a result of these ' activities are described below. SAMPLE IDENTIFICATION The process of selecting an adequate sampling of items to be reinspected involved compiling a list of all major items supplied by BB/MM to Diablo Canyon. The requirements for each item to be verified by BB/MM's quality assurance program were identified. These requirements were grouped into two general categories: (1) compliance with material specifications, and i (2) compliance with acceptable fabrication techniques. A list of all major BB/MM-supplied items and their requirements in each of the two categories described above is provided in Table 1. Some of the items on the list in Table I were previously reinspected by PGandE. The scope of these previous reinspections was considered when the current samples were selected. Project engineering reviewed Table i to ensure that the total sample, composed of previously and currently reinspected items, I would be representative of all of the products supplied by BB/MM and would specifically include: Major structural items used in applications important to plant safety.

This selection process is described ,in more detail below.

A review of Table 1 demonstrates that most of the BB/MM-supplied items were used in nonsafety-related applications. In addition, most of the items were made of common grade, easily weldable materials (e.g., A36 steel). The reinspection sample includes some of these common grade materiais used in safety-related applications. Of more importance were the relatively few items that required the use of higher strength materials (e.g., A441 and A572 steel) which are also readily weldable. Many of these items were included in the reinspection sample.

4 One of the most frequent uses of high-strength steel-(A490) supplied by BB/MH was for anchor bolts. A large sampling of these bolts was made. Another frequent use of BB/MM-supplied special steels was for pipe rupture restraint rods (A276 type 304). Extensive reinspection-of these rods was previously performed, so they were not included in the current reinspection.-

The primary fabrication activity requiring quality verification was welding.

Review of Table 1 shows that most of the heavy welding performed by BB/Mt1 was , in the Class I pipe rupture restraints. These rupture restraints were the subject of a previous reinspection (1979-1980). Welding of the non-Class I structural components enclosing the Condensate Polishing System and Reverse Osmosis System was also previously reinspected (January 1984). A sampling of the welding in the few remaining heavy weldments was included in the current program. Also included in the current reinspection program was a sampling of numerous smaller weldsents. In addition to the items described above, four bulk material items were i included in the sample. These samples are from the Class I bulk materials for 4 which a former Bostrom-Bergen (BB) inspector, Mr. George Dossey, alleged that he falsely documented inspections. i After selecting items as described above, additional items were reinspected to include samples from all time spans during which materials were being processed at BB/MM. This was done to ensure that, if changes in BB/MH's quality assurance practices affected the adequacy of materials supplied during a specific time span, the reit.spection sample would include examples of these items. Three distinct time periods were determined to be significant for products supplied to Diablo Canyon. The first time period, 1969-1976, was when Bostrom-Bergen was a subcontractor to Guy F. Atkinson (GFA), the Diablo Canyon general contractor. During this period, the quality requirements imposed on BB were through GFA purchase orders and correspondence. The second time

!      period, 1977-1979, was that period in which BB was contracted by H. P. Foley 1

(HPF) and Pacific Gas and Electric Company (PGandE). Work done during this period was performed under the purchase order / specification requirements of both HPF and PGandE. The distinctive feature of this time period was that BB utilized its subsidiary Meddco Metals facility to perform a large portion of the fabrications, principally in the manufacture of rupture restraints. The Meddco Metals work during this time was performed under the BB quality assurance program utilizing BB QA and QC staff. The third time period, 1980-present, was established to distinguish the time in which the work performed by Meddco Metals was under its own quality assurance program, both as a subcontractor to BB and as a direct contractor to PGandE. Histograms depicting the correhtion of the quantities (and completeness) of the items sampled in the current reinspection program to the quantities of materials produced at BB/MM are provided in Figure 1. These histograms have been normalized (drawn to a comon scale) to show that the relative density of work processed through time at BB/MH has been reflected in the reinspection program. Figure 2 provides a summary of all the BB/MM materials that were reinspected in both previous and current sampling programs. In summary, most of the material supplied by BB/MH was common grade and was used in nonsafety-related applications. Much of the special steel and heavy 0641d/0005K ___

welding had been previously reinspected. Thus, the current reinspection sample was composed of pieces selected from the relatively few remaining items and provides a representative sampling of the various grades, fabrication processes, time frames, and structurally important applications of the BB/MM supplied materials. REINSPECTION METHODS Reinspection of the selected BB/MH items included verification of both sterials and fabrications. Material verification was accomplished primarily by hardness testing. Hardness readings were taken in the field using an Equotip testing machine. This test provides an indirect measurement of a material's tensile strength. In a few cases, further confirmation of a .

         . material type was achieved by (1) removing a portion of the. sampled item, (2) repeating the hardness test in a laboratory using a full-size Rockwell Hardness Tester and (3) performing a chemical analysis on the removed specimen.

Fabrication verifications were accomplished by: (1) general inspections, (2) visual weld inspections, and (3) nondestructive examination (NDE) weld inspections. General inspections compared existing assemblies to shop drawings to verify geometrical and dimensional conformance. Visual inspections were performed on the welds made by BB/MM and were verified in accordance with AWS criteria. For items requiring heavy shop welding, existing NDE tests performed by an independent laboratory were reviewed. The reinspection test methods are further described in Appendices 1 and 2 to this report, which are sumaries prepared by Bechtel's Materials and Quality Services (M&QS) Department. M&QS personnel performed the hardness and l chemical analysis tests as well as the visual weld inspections. R INSPECTION FINDINGS A total of 112 individual items (or assemblies) were reinspected in the current program. Table 2 provides a list of these reinspected items, the specific tests performed on each item, and the results of the reinspections. Previous reinspections addressed the rupture restraint rods, the BB/MM rupture i restraint welds, and the Condensate Polishing System / Reverse Osmosis System component welds. Figure 2 briefly summarizes the extent of all of these BB/MM material reinspections. l l Material Verification In the area of material verification, common grade steel (A36), high-strength steels (A441, A490, and A572), and stainless steels (A240, type 304) were reinspected in the current program.* A36 steel was frequently supplied for embeds (plates) and beams (structural shapes). Hardness tests were performed on twenty A36 plates and sixteen A36 structural-shapes. ' Hardness tests were also performed on two A572 beams, two A441 beams, nine A441 plates, and two A240 plates. In every case, the hardness tests indicated that the specified material had been provided. In addition, specimens were removed from three stainless steel plates and were subjected to chemical analysis. All three specimens were found to meet ASTM chemical requirements. Appendix 2 to.this report contains the data resulting from the stainless steel chemical analyses. 0641d/0005K - u. . i____.______. _ _ _ _ _ - __ __

g- t A total of 58 A490 anchor bolts was also hardness tested. A490 is a high-strength bolt material whose fabrication requires a special process of quenching and tempering. The field hardness tests were performed on the ends of the bolts. The field readings indicated that high-strength steel had been l provided, but some of the readings were outside of the range given in the ASTM A490 specification. Further investigation was performed by removing a small specimen from the ends of six bolts. The six specimens were selected to envelope the range of field hardness readings. ASTM recommends that hardness tests be made at one bolt diameter from the end of the bolt. Due to the short projection of the bolts beyond their nuts, this was not feasible and the , specimens were cut approximately 1/8" from the ends of the bolts. The'six specimens were subjected to hardness tests in the M&QS 1aboratory. The spread in the readings narrowed and all were in or near specification requirements,. It was judged that, since the hardness readings were very close to A490 requirements, if the specimens had been taken at the one bolt diameter depth, the hardnesses would be within the specified limits. Additional confirmation of conformance to A490 specifications was then achieved by performing an l analysis of the chemical composition of the six specimens. All six specimens ' met the chemical requirements of the A490 specification. Thus, upon

evaluation of all compiled data, it was concluded that the bolts were made of acceptable A490 material. Appendix 1 to this report provides a plot of the distribution of the field and laboratory hardness readings and provides i additional information on the A490 bolt testing and data evaluation.

Previous reinspections of BB/MM-supplied material included review of the U-bolts and straight rods used for the pipe rupture restraints. The material used was specified to have a yield strength between 34 and 46 ksi, which was required to achieve the ductility necessary to absorb the energy created by pipe rupture loading. The rods that left BB/MM's shop met all specification requirements. In 1982, project engineering initiated a testing program to confirm that the actual ductilities of the installed rods, as measured by l hardness testing, were adequate. All 842 rods installed in Unit I were ~ hardness tested. 3ased on the test results, rods found to be outside the required hardness range were solution reannealed and retested. Rods which did 1 not achieve the required yield strength after reannealing were replaced by new rods. NCR No. DCl-83-EN-031 was written against these rods. The excessive , l hardness found in some of the rods was attributed to the fabrication sequencing that occurred at the steel mill (not at BB/MM). Hardness testing was not a specification requirement and thus, there was no error made when BB/MM's QA program accepted the rods. , i Fabrication Verification I

                -In the area of fabrication verification, general conformance to shop drawings      ,

was reviewed for 86 of the items sampled in the current program. All were l t found to be in conformance with the shop drawings. A previous reinspection of l 1 the BB/MM-supplied pipe rupture restraint rods, performed in 1982 and 1983, reviewed 439 straight rods and nineteen U-bolts for straightness and' dimensional conformance to design drawings. All were found acceptable. i Visual inspections were performed on the 22 assemblies in the current sample , that contained shop (BB/MM) welds. AWS visual weld inspection criteria, as l outlined in Bechtel Standard VE-AWS, were used for this reinspection. This l 1 standard was attached as Appendix B to the interim report (PGandE Letter l - No. DCL-84-081). The weldments visually inspected generally contained small 0641d/0005K j l 1

fillet welds in attachments to structural beams, jet impingement barriers, and ! platforms. Although a few welds had some very minor indications, all of the welds were found to meet design requirements. Most of the heavy welding performed by BB/MM was in the pipe rupture restraints, which had been previously reinspected. An initial sampling of the BB/MM pipe rupture restraint welds was reinspected by MT and UT examination. i The reinspections were performed in 1979 and 1920 when the restraints had been installed in the plant. From the initial sample, engineering identified all shop welds which required further examination. Although some defects were . found in the BB/MM welds, subsequent engineering evaluation found them to be , acceptable. Further investigation found that the defects were related to a (qualified) BB/m welding procedure which was used only on the fabrication.of

the rupture restraints. An NCR, No. DCl-79-RM-010, was written against these t welds and the deficiencies were appropriately reported to the NRC per
;              10 CFR 50.55(e) on May 3, 1979. Review of the BB/MM welding procedures determined that the defects could have occurred at any time (i.e., either before or after shipment to the jobsite). In order to confirm that the BB/MM quality assurance requirements had been properly executed, the shop-weld NDE records were recently reviewed. This review confirmed that the required NDE

! had been performed by independent laboratories. These NDE reports indicated that the defects did not exist when the assemblies left BB/MM's shop, thus, there was no breakdown in BB/MM's quality assurance program. The previous reinspection of the BB/MM welding in the Condensate Polishing

System and the Reverse Osmosis System components is described in Section 4.1 of PGandE Letter No. DCL-84-051, dated February 10,19B4. This reinspection

! sampled 58 piece marks containing 940 individual welds that were directly related to Mr. Dossey's allegations. This reinspection, performed in , January 1984,.confirued the adequacy of the subject BB/MM welding. The previous reinspections described above addressed most of the heavy welding performed by BB/MM. Of the few remaining heavy weldments, three were addressed in the current sampling program. These were in the reactor support beam, auxiliary building jet impingement barrier beams, and the component . cooling water heat exchanger barrier beams. The PGandE specifications and the BB/MM shop drawings for these assemblies specified that UT and MT NDE was required. The corresponding inspection. records were reviewed. The records l indicated that the required UT and MT were performed and that the welds were found to be acceptable. The NDE technician who performed the weld i examinations was a qualified Level II inspector whose qualifications are on file. CONCLUSION i As a result of allegations regarding the quality assurance practices used at BB/MM, extensive reinspection of the BB/MM materials and their fabrication was performed. Although previous reinspections identified a few deficiencies in the BB/MM-supplied materials, these were not attributable to a breakdown in i the BB/MM quality assurance program. All of the deficiencies have been corrected by PGandE. For every item sampled in the current program, reinspection showed that acceptable materials and fabrications were provided to Diablo Canyon. All field hardness testing was performed using the EQUOTIP Hardness Tester in accordance with the attached procedure (Attachment I).

i Field testing consisted of preparing the test area by surface grinding followed by l buffing with a 60 grit flapper wheel to produce the surface finish required for the EQUOTIP Hardness Tester. A minimum of five readings were taken in each area. High and low values were dropped, and the three remaining values were averaged. The averaged EQUOTIP readingsD(L ) were converted to either Brinell hardness (BHN) or Rockwell C (ERC) hardness valuem using the manufacturer's conversion t ab les . The test data have already been given to the project. Several types of structural steel were examined. All members examined had

                       'hardnesses expected of their respective type and grade. The A36 structural members had a hardness range of 112,to 162 BHN, with the majority of readings falling between 120 and 140 BHN. Ttso A572 members were tested , and the results were 149 BEN and 161 BEN. Four testa were performed on A441 type material, j                        resulting in 144 to 155 BRN. The higher strength grades of steel displayed higher and more consistent hardness values, indicating higher strengths. No l

abnormalities were noted, and we find no evidence that the materials are other j than as required by specification. l Fifty-eight bolts were examined. The drawings specify high-strength ASTM A490 l quenched and tempered bolts. The testing was performed in order to provide additional assurance that A490 type materials were installed instand of low-strength fasteners (A307 type). Material Verification of March 9,1984 Structural and Bolting Diablo Canyon Project, Unit 1 Bechtel Job No. 15320-002 The majority of the samples tested conformed to the hardness requirements of ASTN A490-83 (33 to 38 BRC). See Figure 1 for a summary of fleid hardness test results. Several of the bolts produced large deviations from A490. These samples were reground and buffed as before, with more emphasis on the surface preparation (flatter, smoother finish, etc.), then retested. This resulted in more favorable readings but still produced some high and low readings. A small piece was cut ~ from the end of each of the'six talts in question, and they were brought to the M6QS laboratory for further examination: two samples were cut from bolts with the lowest hardness readings, one sample at the lower ASTM A490-83 limit, and three with high hardness readings. These samples were prepared for hardness testing (using a full size laboratory Rockwell Hardness Tester) and chemical analysis. The laboratory test results are shown in Tables 1 and 2. A chemical analysis showed that the six samples meet the chemical requirements of A490 type bolts (Table 2). ASTM A490 has chemical requirements only on carbon, ~ phosphorum, and sulfur. Other elements are added to enhance the hardenability and strength levels of the material for large diameter bolts. The hardest sample (approximately 53 BRC) removed for laboratory testing was tested at a depth of approximately 1/8 inch below the original surface to avoid any end effect from handling or poor grinding technique. It gave Rockwell C readipgs within ASTN's limit for A490 bolts. The sample that gave field values of 45 BRC was first tested on the original surface. Laboratory HRC values were still out of specification. This sample was retested at a depth of approximately 1/8 inch below the original surface, and the hardness level dropped 5 HRC points. The remaining bolt samples tested also moved into or close to the hardness range required by ASTM A490-83. The original hardnesses of the six specimens are noted l in Figure 1. Laboratory hardness test results at approximately 1/8-inch depth are i also presented in Figure 1. Laboratory hardness readings were taken at the mid-radius location, as required by ASTM A370, " Mechanical Testing of Steel Products." The only deviation from ASTM requirements was that readings should have been taken at depth below the original surface equivalent to the diameter of the bolt. This was not possible bc:ause only a short piece could be cut from the end of each bolt. l Some slight scatter of hardness data beyond specification limits was found and was expected for the following reason: The RQUOTIP Hardness Tester only makes a shallow indentation. Typically, hardness indenters that have shallow penetration depths not only provide more data scatter, but they also tend to provide harder readings since they are more sensitive to small variations. A deeper indenter (Brinell, HRC, etc.) tends to average minor area variations.

Laboratory EQUOTIP hardness testing was performed adjacent to the Rockwell C  ; readings on both the field. exposed end and at the 1/8-inch depth. The results , i presented in Figure 2, are compared to the manufacturer's conversion table (y l J to ERC). Rockwell and gQUOTIP hardness testing produced the same results on both the field exposed end and at the 1/8-inch depth with the expected amount of scatter. That data plotted are also presented in tabular form. This reestablishes the validity of the EQDOTIP Hardness Tester and the manufacturer's i conversion tables.  ! i Based on the field and laboratory hardness testing, it has bee'n shown that high- l l strength, low-alloy steel bolting has been installed in conformance with the l engineering requirements. All field hardness testing was performed using the Equotip Hardness Tester in accordance with the procedure - Equotip Hardness Testing Procedure HT P-1 Rev. O. Hardness testing consisted of preparing the surface by surface grinding followed

          - by buffing using a 60 grit flapper wheel to produce the surface finish required byifTP-1. A minimum of five hardness readings were taken in each area. High and low values were dropped, and the three remaining values were averaged. The averaged Equotip readings (LD) were converted to Brinell hardness (BHN) using the manufacturers conversion tables.-

All of the structural members tested were specified to be A36 and A240 Tp 304 material. All members had hardnesses expected of their respective type and grade. A36 hardness values ranged from 120-174 BEN with the majority of readings falling between 120 to 137 BHN. Stainless steel sabeds had hardness values from 171 to 191 BILL . The samples cut from stainless steel imbeds for chemeial analysis were specified to be A240 Tp 304 material. The results of the analysis is shown in Attachment I compared with ASTM's chemical requirements for A240 Tp 304 stainless steel. Based on these results Materials and Quality Services (M&QS) recommends th'at all materials tested be accepted as-is and that no other action be taken. G. R.
