10. NON-CONFORMITIES 10.1 AT JOBSITE 10.1.1 The Field Q.A./Q.C. Manager is responsible for assuring that all necessary controls are implemented to prevent use of non-conforming items during site fabrication and installation.

10.1.2 All non-conformities shall be tagged by a field Q.A. or Q.C.

Inspector with a Hold Tag and placed in a holding area (when-ever practical) awaiting action. When not practical, a Hold Tas shall be sufficient. The Q.A./Q.C. Manager shall have the option to determine if a hold tag is required.

10.1.3 The Authorized Nuclear Inspector shall be consulted in ad-vance of all material repairs.

10.1.4 All non-conformities shall be reported on a Discrepancy m

report (D.R.) (Fig. 6) and submitted to the Field Q.A./Q.C.

Manager for his approval.

10.1.5 The Field Q.A. or Q.C. Inspector and the Field Engineer will outline corrective action to be taken on the D.R.

The customer will review all D.R.'s and either approve the Company recomendation or add their coments.

The D.R. shall not be issued until the D.R. is approved by the customer and the Field Q.A./Q.C. Manager.

A.

Following the approval of the D.R. by P.G.&E., the Field C.A./Q.C. Manager shall notify the Authorized Nuclear In-spector of the disposition by sending him a copy of the approved D.R.

B.

The disposition may be: use as is, repair or scrap. When there is a repair, the repair will be made 'in accordance with the recomended disposition on the D.R.

C.

The repaired item will be, inspected by a Field 0.C.

Inspector who documents the required inspections on the applicable process sheet.

D.

Any changes in fullfillment of the approved recomended disposition will require re-suomittal and re-approval of the D.R. A " Hold" tag must be re-appplied while waiting for,

the revised Discrepancy Report.

If scrapping is recomended, the Field Q.A.or 0.C. Insnactor shall indicate this on the " Hold" tag. All scrappeo material shall be imediately deposited in an area set aside for this purpose.

When the recomended work and documentation is com-pleted, the Field Q.A. Inspector (Auditor) will notify the Field Q.A./Q.C. Manager that the 0.R. is ready to be closed. The Field Q.A./Q.C. Manager will close the D.R.

by initialing the Final Disposition and will have the completed D.R. filed in the Master Job File.

Veld Parameters: Welding parameters are spect fled in the table on page 3 of 3 Welding Technleve: The welding technique shall be as follows:

A) BEAD WIOTH - All welding will be acccmolished using the stringer bead

. technique. Weaving is allowed on the cover pass only to a maximum of 5 times the electroda diameter.

INTERPASS CLENilNG - All weld beads will be f ree of all slag prior to continuation of welding. NEEDLE GUNS shall not be used for any cleaning operation.

C) DEFECTS - All visable defects, i.e., porosi ty, crater cracks, cold lac, shall be removed prior to the continuation of welding by grinding or or filling.

D) RUN-OFF TASS AND BACKING STRAPS - Run-off tabs and b$cking straps will be used whenever possible.,Run-off tabs should be removed. Backing straps need not be removed *unless specified by the owner or his desig-nated representative. Remeval of run-off tabs shall be accompli 1neri sy thermal cutting wi thin 1/8" of the weld ~c.t and then blended into the

base metal by grinding. Alternatively, removal naay be entirely accom-plished by grinding. Removal of backing straps will be accoinolished by grinding or gouging to sound metal, and then back welded as needed.

When any* thermal process is used, the applicable pecheat requirements

TO All QC INSPECTORS FROM HAROLD KARNER SUBJECT FINAL AS-BUILT INSPECTION Effective December 12, 1983 in Unit #1 we will resume final as-built it.spection with reviewing the approved for construction drawing and all applicable documentation as was the case prior to August 27, 1983.

This memo rescinds that memo issued by me on August 27, 1983.

Due to the high rejection rate being found by QA final review, I am looking to you to help decrease this by doing a rough, but close documentation review to assure all proper and necessary documentation is enclosed.

In order to help boost the efficiency rate of our' final 4

inspections in both Unit #1 and Unit #2 you will find attached an inspection checklist to be incorporated in j

all your final as-built inspection packages.

This check-i l

list is provided to act as a guide and a reminder during j

your inspection, and will enable you to render a more complete inspection.

As you perform your final inspections you will to down the caecklist and initial and date each block applicable to your installation.

If an item is not applicable you will insert an "N/A".

All blocks will either be "N/A'd" or initialed.

At the completion of your inspection you will sign off the signature block on page three and turn it in with the final package.

These will then be. removed and kept by your supervisor prior to the package being sent to QA Review.

If two or more inspectors inspect the same support, there will be separate checklists attached and both lists will accompany the package.

You may also use this checklist as a guide during the workmanship stage of the support.

However, you will not have to incorporate it into the package nor will you have to sign it.

4) Do'not drag leads over pipes, ete: if they have bare spets or open connectors they may arc.

All employees are urged to follow approved plant procedures and plant technical specifications. In doing so, the plant staff will meet all governmental rules and regulations, including those issued by the Nuclear Regulatory Commission (NRC). Should any employee feel that such rules or regulations are not being followed, he should pursue the matter with his supervisor. If the matter cannot be resolved through his supervisor, he should pursue it via the "Open Door Policy", which has been previously stated and is repeated ort the following page. Should any employee feel that NRC rules or regulations are not being followed and plant management is not correcting the situation, NRC regulations allow him the right to discuss the problem with NRC inspectors (see'NRC-3 notice posted on the bulletin board). Two NRC inspectors, Marvin Mendonca and Mark Padovan, are located in the office trailer adjacent to the Administration Building (plant extension 2439). Should an employee prefer not to talk to the onsite NRC inspectors, the Region V NRC office address and tele-phone number is: U.S. Nuclear Regulatory Comission Region V 1450 Maria Lane Walnut Creek, CA 94596-5368 (415) 943-3700 This memorandum does not authorize any employee to leave his assigned job or interrupt his work assignment to meet with NRC inspectors during work hours. It is preferred that, if necessary, employees contact NRC inspectors during nonwork hours. However, if a meeting must take place during work hours, an employee may request permission from his super-viscr. Any employee who requests time to meet with NRC inspectors will be granted such time off at a scheduled, convenient time.

~~ TO'ALL EMPLOYEES February 5, 1982 OPEN 000R POLICY I would like to take this opportunity to reiterate our policy concerning-the employee's right and obligation to inform minage-ment of safety concerns. Any employee who feels he has identified a concern or problem affecting plant safety or reliability should discuss this matter with his imediate supervisor. Following such a discussion, if the employee feels that his concern is not being adequately addressed, he should pursue the matter with progressively higher levels of supervision, up to and including the Plant Manager. While it is desirable to keep minor complaints from reaching this level of management, I would like.all employees to feel free to approach the Plant Superintendent and the Plant Manager with his concern if he feels it is necessary. Voicing concerns regarding plant safety or operation will not be documented in the employee's personnel record and will never be used for any type of disciplinary action. I give my firm guarantee of that. This policy also applies to any situation-in which the employee feels he is being requiied to do any work which would jeopardize his personal safety. ~ PGandE QUALITY HOTLINE I wish to call attention to signs which have been posted around the plant and project concerning the " Quality Hotline".
