APPENDIX A NOTICE OF VIOLATION Connecticut Yankee Atomic Power Company Docket No. 50-213 Haddam Neck Plant License No. DPR-61 During an inspection from January 11-21, 1988, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C) the following violation was identified.

Connecticut Yankee Atomic Power Company Haddam Neck Plant, Technical Specification 6.8. "Procedures", requires that written proceduras and administrative policies shall be established, implemented and maintained that meet the requirements and recommendations of Sections 5.1 and 5.3 of ANSI N18.7-1976 and Appendix A of USAEC Regulatory Guide 1.33.

Section 6.1.1 of Surveillance Procedure No. SUR 5.7-67, Inservice inspec-tion of Service Water Isolation Valves SW-A0V-129, SW-A0V-130, SW-A0V-8 and SW-A0V-9, Revision 8, requires recording of the opcning time for Service Water Valve SW-A0V-129.

Contrary to the above, on January 12, 1988, the opening time for Service Water Valve SW-A0V-129 was not recorded during the above surveillance tests This is a Severity Level V Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Connecticut Yankee Atomic Power Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending this response time.
