MPENDIX A NOTICE OF VIOLATIQH Connecticut Yankee Atomic Power Company Docket No. 50 213 Hartford, Connecticut L.icense No. DPR-61 As a result of the inspection conducted from November 16-20,1992, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Act'ons " 10 CFR Part 2, Appendix C (1992), the following violations were identified:

A. 10 CFR 50.49, Paragraphs f and g require that electric equipment important to safety be environmentally qualified by type testing, andysis, or a combination of both, and that the qualification must be accomplished before November 30,1935.

Contrary to the above, on April 20,1992, the qualification of salenoid valves CC-SOV-608 and CC-SOV-1411 were not established in that these two valves were not in Haddam Neck Environmental Qualification (EQ) Master List, and that no EQ files containing test or analysis occuments were established to demonstrate the qualification of these valves, i

'?his is a Severity Level IV Violation (Suppleme:e I).

B. 10 CFR 50, Appendix B, Criterion V, requires that activities affecting quality shall be prescrital by documented instructions and shall be accomplished in accordance with these im.tructions.

Design Change Notice DCN-407-E-89 specifically identiSed that 2/0 jacketed ground cabic (C11T348) 'o be connected to the base of electrical penetrations.

Contrary to the above, on November 19,1992, the above cable was found to be connected to the duct bank ground grid.

This is a Severity Level V Violation (Supplement i).

For Violation A above, the corrective actions have been reviewed by the NRC and detennined to be adequate. No response is required. For Violation it, pursuant to the provisiens of 10 CFR 2.201, Connecticut Yankee Atomic Power Company is heieby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commhsion, ATTN: Document Comrol Desk, Washington, D.C. 20555 with a copy to the Regiona!

Administrator, Region I, and a copy to the NRC Resident Inspector, within thirty days of the receipt date of the letter which transmire 'his Notice of Violation (Notice). This reply should be clearly marked as a " Reply i a lotice of Violation" and should include: (1) the reason for the violation, or, if contested,, ute basis for disputing the violation; (2) the corrective steps that have been taken and the rentlts achieved; and (3) the corrective steps -

which have been taken or will be taken to avoid funher violations. Where good cause is shown, consideration will be given to extending this response time.
