APPENDIX A NOTICE OF VIOLATION Connecticut Yankee Atomic Power Company Dacket No. 50-213 Haddam Neck Plant License No. DPR-61 As a result of the inspection r.onducted on August 29 - October 31, 1984, and in accordance with the NRC Enforcement Policy, 10 CFR 2 Appendix C, the following violations were identified:

I. Technical Specification (TS) 3.22.F.1 requires penetration fire barriers, in-cluding cable penetrations of safety-related areas, to be functional or to be monitored by a fire watch. Pursuant to the above, Procedure 8.5-143, In-stallation, Repair and Inspection of Fire Barrier Penetration Seals, requires a fire watch to be maintained whenever a penetration is opened.

Contrary to the above:

1. On or about August 1,1984, Penetration Nos. 800 and 801 between the safety-related cable vault and the auxiliary feed pump room were found to be non-functional in that they were not sealed and no fire watch had been established; and
2. On October 30, 1984, the temporary fire seal on Penetration No. 800 had been removed and no fire watch had been established.

This is a Severity Level IV Violation (Supplement I).

II. 10 CFR 50, Appendix B, Criterion III requires that design changes, including field changes, be reviewed by the organization that performed the original design unless the applicant designates another responsible organization. The Northeast Utilities Quality Assurance Program specifies design control meas-ures which conform to the recommendations of Regulatory Guide 1.64 and ANSI N45.2.11, Quality Assurance Requirements for the Design of Nuclear Power Plants. These standards specify that changes to design documents be reviewed and approved by the same groups or organizations that reviewed the original docum9nt. Nuclear Engineering and Operations (NE&O) Procedure 3.03, Prepara-tion, Review and Disposition of Plant Design Change Requests, and Generation Engineering and Construction (GE&C) Procedure 2.01, Preparation, Review, Ap-proval, Revision and Control of GE&C Division Specifications, specify controls for the preparation, review, approval, and revision of design documents.

Contrary to the above, field changes to design documents are not subject to the same review and approval process applied to the original design, in that field change control procedures do not implement: (1) the quality assurance 8412180425 841212 DR ADOCK 05000g33 l PDR OFFICIAL RECORD COPY DL50-213/84 0003.0.0 11/19/84

Appendix A 2 and supervisory reviews applied to design specifications by GE&C Procedure 2.01; and (2) the design verification, committee review, and management ap-proval of design changes required b) NE&O Procedure 3.03.

This is a Severity Level V Violation (Supplement I).

Pursuant to 10 CFR 2.201, Connecticut Yankee Atomic Power Company is hereby re-quired to submit to this office, within thirty days of the date of the letter which transmitted this Notice, a written reply including: (1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
