- l l NOTICE OF VIOLATION Connecticut Yankee Atomic Power Company Docket No. 50-213 Haddam Neck Plant License No. DPR-61 l i During NRC inspections conducted from November 16,1996 - January 5,1997, a j violation of NRC requirements was identified. In accordance with the " General Statement j of Policy and Procedure for NRC Enforcement Actions," (Enforcement Policy),

i NUREG 1600, the violations are listed below:

Technical Specification 6.8.1 requires, in part, that written procedures shall be established, implemented, and maintained covering the activities as recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978. Regulatory l

Guide 1.33 requires procedures to provide instructions for maintenance and testing ,

} the emergency power systems. Preventive Maintenance Procedure (PMP) 9.1-31,

!- Diesel in-Leakage and Fuel Oil Transfer Pump Availability, Revision 11, dated January 23,1996 was prepared pursuant to the above, and in Step 6.2.10 required the operator to remove the "B' emergency diesel generator (EG-2B) crank over tool and to place it in its storage rack.

! Contrary to the above, on November 27,1996, during the preparations to test EG-2B after jacking the engine, the operator failed to remove the crank over tool per j Step 6.2.10 prior to running the diesel for the one hour operability test.

This is a Severity Level IV violation (Supplernent 1).

i l Pursuant to the provisions of 10 CFR 2.201, Connecticut Yankee Atomic Power Company j is hereby required to submit a written statement or explanation to the U.S. Nuclear j Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555, with a copy to the Regional Administrator, Region I, within 30 days of the date of the letter j transmitting this Notice of Violation (Notice). This reply should be clearly marked as a l " Reply to a Notice of Violation" and should include for each violation: (1) the reason for j the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous docketed correspondence, if the l correspondence adequately addresses the required response. If an adequate reply is not l' received within the time specified in this Notice, an order or a Demand for Information may (

be issued as to why the license should not be modified, suspended, or revoked, or why

. such other action as may be proper should not be !aken. Where good cause is shown, consideration will be given to extending the response time.
