APPENDIX A NOTICE OF VIOLATION Connecticut fankee Atomic Power Company Docket No. 50-213 Hartford, Connecticut 06101 ' License No. OPR-61 As a risult of the inspection conducted on February 10-14, 1986, and in ac-cordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations were identified:

Technical Specification 6.8 states, in part, that temporary changes to prccedures may be made provided the intent of the original procedure is nct altered, the change is approved by two members of the plant management staff, at least one of whom holds a Senicr Reactor Operator's License, the change is' documented, reviewed by the Plant Operating Review Committee (PORC), and approved by the station superintendent.

Section 6.3.2 of Procedure No. RAP 6.3-5 requires that solid waste be compacted in a compacting box and tha.t'a lid be installed to seal the box prior to the compacting operation.

Contrary to the above, on 5 September, 1985, a highly contaminated drum was compacted without the use of a compacting box or sealing lid, result-ing in extensive contamination of the compacting area and internal contam-ination of three workers. This operation was an alteration of the origi-nal intent of the procedure but the, change was not approved by plant management, documented in accordance with procedures, reviewed by the Plant Operating Review Committee, and approved by the station superinten-dent. -

This is a Severity Level IV violation. (Supplement V)

Pursuant to the provisions of 10 CFR 2.201, Connecticut Yankee Atomic Power Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted'this Notice,' a written statement or ex-planation in reply, including: (1)' the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the jete when full compliance will be achieved.
