SUBJECT:
SALEM UNDERVOLTAGE ATTACHMENTS Your April 15, 1983 memo to me suggested two actions the staff should take concerning tests conducted on the UV attachments that failed during the February 22 and 25,1983 Salem events.
First, you suggested that the staff attempt to verify it has correctly identified the UV attachments that failed on February 22 and 25.
Second, given that the staff has correctly identified the failed UV attachments, you suggested that the NRC obtain one, determine what maintenance, modifications or tests have been conducted on the failed UV attachment, and then provide the attachment to one of our contractors for examination.
As part of our continuing review, the licensee was sent a 10CFR50.54(f) letter dated April 20, 1983 (enclosed).
That letter required, among a number of other items, that the licensee confirm the disposition of the failed UV attachments. The licensee responded, by letter dated April 22, 1983, (enclosed) that the failed UV attachment from Reactor. Trip Breaker (RTB) B was g.iven to Westinghouse intact on February 28, 1983. The licensee also stated that the failed UV attachment from RTB A was given to Westinghouse disassembled on March 2,1983.
Both UV attachments that failed, as identified above, were sent from Westinghouse to our contractor, Franklin Research Center (FRC), on April 15, 1983. The results of FRC's and the staff's examination of these devices, as discussed at the April 26, 1983 Commission meeting, were consistent with results of the examination performed by FRC on the UV attachment previously provided tq t. hem.
