This refera to the telephone conversations between Messrs. Dodds, 1

Vorderbrueggen and Haist of this office with you on Hay 8,1979, and l

with Mr. Hand of your staff and Bechtel project personnel on May 9,1979, regarding severe misfit and distortion problems associated with the installation of the Unit 1 containment dome liner segments and the current field procedures to make force fit-ups. As you know, these problems were discussed on May 4,1979 during our construction inspec-tien at the Palo Verde plant site.

In this regard, it is our uncerstanding that you are taking the following actiens:

1.

Work has been sushended on dome segments of Unit 1 until it has been determined that appropriate remedial measures can be implemented to prevent high residual stresses frem being induced in the finished welds on the liner plate, and these remedial measures have been.

reviewed with Region V.

2.

For Unit 2, it is understcod that no erection or weldian of 30 degree sencents will be comnleted on the containment dome until the prob-lem has been resolved and reviewed with Region V.

l 3.

Structural specialists from the Bechtel 1.os Angeles office are L:

exaufning the situation at the work location and will identify the l

activities necessary to resolve the existing conditions, and the procedural measures needed to prevent reoccurrence of a similar L

condition on Units 2 and 3.

COMMENT:

The OD0's response indicated that the condition was previously reference.! on various Docu.nent Deficiency Notices in other packages. It appears that not all the packages with the subject material were noted. But the ODO stated that a Non-Conformance Report will be generated, and all remaining packages referencing the use of the material will be noted on the Non-Conformance Report.

This' is a satisfactory method to handle the problem.

The response only included plans to change the PQAM which contains inappropriate reference s to design reviews. After review of the OLO response, a telephone conversation was' held with Mr. Nicodemus of PSI. In that conversation Mr. Nicodemus indicated that the procedure on PSI reviews of FCR's could also be clarified..Mr. Nicodemus stated he would initiate action to change the procedure regarding PSI engineering reviews of FCR's. Making the change will clarify the responsibilities in the design review process.

DESCRIPTION OF POTENTIAL FINDING: Available records do not demonstrate that Grinnell's welders are qualified to the weld procedures called out on the shop traveler. It appears that Grinnell's Welding Procedure Specifications (WPS) include several essential variables, which would nomally require a separate WPS. However, in lieu of a separate WPS, several Procedure Qualification Records (PQR) were made for each WPS. Each PQR establishes the essential variables for each process. The concern stems frcra the way weld procedures are called out on shop travelers.
