8301-WTP-1-Q Revision 2 Page 3 of 7 Procedure for Documentation of Postulated Interactions I. SCOPE This procedure describes the method of documentation and control of postulated interactions identified during plant walkdowns.
II. RESPONSIBILITIES' The Walkdown Team Leader shall be responsible for ensuring that interactions are documented in accordance with this procedure.
III. DEFINITIONS A. Interaction Identification Sheet (IIS) - The IIS is the document which describes the interaction and contains resolution information. (See EXHIBIT A)
IV. PROCEDURE A. Postulated interactions are identified by the walkdown team in accordance with the Spatial Systems Interaction Program / ~
Seismic (SSIP/S) criteria and procedures.
B. Interaction Identification Sheets (IIS's) are used to document interactions identified during SSIP/S walkdowns.
Minimum documentation requirements for inclusion on the IIS at the time of walkdown identification include:
- 1. Unique numbering of-each interaction consisting of the room number and a seque'nce number.
- 2. Location of each interaction in the plant
- 3. Identification of the interacting source and target components

- 4. A physical description of the interaction
- 5. A recommended resolution
- 6. Identification of responsible discipline for resolution
- 7. Signatures of the walkdown team member and leader
8001-WTP-1-0 Revision 2 Page 4 of 7 The ab'ove required minimum documentation shall be included in the initial issue (Revision O) of sheet 1 of the IIS.
C. Interaction Identification Sheets are used to document and summarize subsequent actions during the resolution phase of the identified interactions, and include:
- 1. Final resolution / verification - a brief description of the final interaction resolution is provided. The resolution may differ from the resolution recommended by the walkoown team on sheet 1. The MGJEC SSIP/S Project Manager's signature indicates the acceptability of analytic work supporting the interaction resolution.
- 2. Field verification - when the final resolution includes a plant modification, the Walkdown Team Leader shall endorse the IIS indicating that the installed modification satisfactorily resolves the identified interaction and that no new interactions have been created.
- 3. Final CPCo approval - upon completion of all required actions, the IIS shall be submitted to the CPCo SSIP/S Project Engineer for approval. Whether interactions are resolved by No Action Necessary or require the use of the IIS sheet 2, the final approval shall be made on sheet 1 of 2. -
The CPCo SSIP/S Project Engineer's signature indicates final, complete and acceptable close out of all interaction resolution and plant walkdown activities associated with the IIS. IIS's shall be issued as a subsequent revision upon endorsement by the CPCo SSIP/S Project Engineer.
D. Selected information contained on the IIS is entered into the SSIP/S computerized data base system which is "for ,

V. REQUIRED DOCUMENTATION A. IIS (EXHIBIT A)
B. Interaction Identification Sheets shall be controlled in accordance with the provisions of ATP-10-0 for contro11ed' documents. Original, completed and signed-off IIS's are maintained in the files of the SSIP/S on-site group at the '
