LIMITED TEST PROGRAM AT ZIMMER, REVISION Based on our August 20, 1981 telephone conversations, enclosed is the revised limited test program that we plan to perform at Zimmer during the period of August 24 to September 4,1981.

Background
The Zimmer investigation has shown that a breakdown in Quality Assurance (QA) occurred in multiple areas during construction.
It has also identified hardware deficiencies.in several areas.
In other areas the breakdown of the QA Program indicates that hardware deficiencies may exist, but specific deficiencies have not been identified, The licensee is performing an extensive Quality Confirmation Program to determine the quality of plant construction.
This program is expected to take about eleven months.
In the period of August 24, 1981 to September 4, 1981, the NRC will undertake a limited program to determine if hardware deficiencies exist in areas where the QA records are missing or are questionable.
The licensee has agreed to provide services under NRC direction and observation to accomplish this program. NRC will select all samples and have them analyzed at Franklin Research in Philadelphia.
It must be pointed out that the failure to identify hardware problems in this limited test program does not mean hardware problems do not exist.
The NRC limited program is presented below.
Material Traceability - Diesel Generator Room 1.
For pipe with no heat number Select three pipes at random and obtain 24 inch samples a.
b.
Take two sections from one pipe c.
Have these four sections analyzed for; (1) Haterial type (2) Material composition (3) Hardness (4) Meta 11ography 2.
For pipe with incorrect heat numbers; a.
Select at least two pipes and take 24 inch samples b.
Analyze as in 1.c.
c.
For other pipe, analyze each with portable analyzer (DRRRI to arrange for analyzer and trained operator) 3.
For all pipe pieces sent to Franklin Research Center for analysis, also analyze each with portable analyzer.

Welding Problems 1.
In an area in which Resident Inspector has identified lack of weld rod control as evidenced by uncontrolled weld rod lying around, identify six welds whose records do not establish weld quality.
2.
Radiograph these six welds.
Radiography to be witnessed by NRC and radiographs to be read by NRC.
3.
Obtain boat sample from each weld and analyze for material composition, hardness, and metallography.
4.
Determine six socket welds whose records do not identify weld quality.
5.
Radiograph these six socket welds.
Radiography to be witnessed by NRC and radiographs to be read by NRC.
Note: Steps 4 and 5 are optional. Do on time available basis.
Nonconformance Report (NR) Voiding 1.
Select the NRs discussed in Section 4.1 of the investigation report.
2.
Make a field examination to determine if each nonconforming condition still exists.
a.
If it does not, determine if corrected condition is satisfactory and what caused corrective action to be done.
b.
If it does, make a qualitative engineering evaluation of the nonconforming condition and determine its effect on plant quality.
