ZIMMER IhTESTIGATION It is requested that the investigation currently in progress at the Zimmer Site, which resulted from the government accountability projects

questions regarding our previous investigation of Mr. Applegate's allegation, include the following:
Documentation of the areas investigated should address:

a.
The allegation The manner in which the allegation was reviewed. List b.
documents reviewed, individuals with whom discussions were held, and direct observations made. For facts determined by conversations with individuals, document the areas discussed and the information obtained.
State the acceptance criteria used by the investigator / inspector c.
in reaching a conclusion regarding the allegation.
d.
Clearly state the conclusion.
Sworn statements will be obtained from those allegers who presented 2.
information to Mr. Applegate. Statements obtained from other persons such as QA/QC inspectors will not be sworn statements, unless the investigator believes this is appropriate.
Since independent tests or radiographs are not intended, please 3.
assure that a determination is made that test results and radiographs are not fraudulent and report the basis for this determination.

Since it has been stated that management statements may not be accurate because they have a vested interest in the site, verify at least a percentage of management statements by such means as records or direct observation to assure their accuracy.
