MISSED SURVEILLANCE - REACTOR BUILDING ANNUAL INSPECTION l DUE TO MANAGEMENT OVERSIGHT TMI-1 was operating at 100% power on May 10, 1993, when it was discovered that Surveillance Procedure (SP) 1301-8.1 " Reactor Building Annual Inspection" had not been performed by the required completion date of April 2, 1993. The root cause of the late surveillance was management oversight due to inadequate tracking of surveillances. The inspection revealed lightning rod damage and lainor corrosion on a limited area of the Reactor Building liner plate. These findings are being scheduled for resolution in a timely manner, however neither have the potential to compromise Reactor Building integrity. This event is reportable in accordance with 10 CFR 50.73(a)(2)(1)(B). Performance of the inspection May 10-12, 1993 revealed no significant structural degradation to the Reactor Building. There were no safety consequences associated with the late performance of the i inspection. GPU Nuclear has developed a mechanism for tracking to ensure the timely completion of surveillances such as SP 1301-8.1. Improvements will be fully implemented by June 30, 1993. i 1

THI-1 was operating at 100% rated power. There were no abnormal conditions or equipment lineups.

11. Status of Structures, Components, or Systems that were Inoperable at the Start of the Event and that Contributed to the Event: ,

None.

III. Event

Description:

In November 1992 the Plant Engineering group received a scheduling notice for Surveillance Procedure (SP) 1301-8.1, " Reactor Building Annual Inspection." The notice was passed on to a newly assigned individual.

Plant Engineering did not follow-up on ensuring the task was completed nor did Plant Engineering include the surveillance on the department's work schedule. On May 10, 1993 it was discovered that Surveillance Procedure 1301-8.1 ' Reactor Building Annual Inspection" had not been performed by the required completion date of April 2,1993.

Plant Engineering took action promptly to perform the surveillance, which was completed on May 12, 1993. The inspection revealed a few broken lightning rods on top of the building and minor corrosion affecting a limited area of the Reactor Building liner (NH/LNR) plate. These findings '

are being scheduled for resolution in a timely manner, however neither of these findings has the potent,ial to compromise Reactor Building integrity.

The root cause of the late surveillance was management oversight. Because Plant Engineering is assigned relatively few surveillances, the system for tracking surveillances did not in this caso provide adequate visibility of surveillances approaching their due date.

The inspection revealed no significant structural degradation. This event ,

is reportable in accordance with 10 CFR 50.73(a)(2)(1)(B), "a condition '

prohibited by the Plant's Technical Specifications.", 1 IV. Component Failure Data ,

V. Automatic or Manually Initiated Safety System Responses: l No safety system responses occurred or were required to occur. l VI. Assessment of the Safety Consequences and Implications of the Event:

Performance of the Reactor Building Annual Inspection May 10-12, 1993 revealed no significant structural degradation to the Reactor Building.

There were no safety consequences associated with the late performance of the Reactor Building Annual Surveillance.

VII. Previous Events of a Similar Nature:

There have been no occurrences of a missed surveillance at TMI-l involving Plant Engineering. LER 91-008 discussed a late surveillance within the Chemistry Department which uses their own tracking program.

VIII. Corrective Actions Planned:

GPU Nuclear has developed a mechanism for tracking to ensure the timely completion of surveillances performed by groups which are responsible for  !

relatively few surveillances. Full implementation of these improvements l will be completed by June 30, 1993.
