1. Current Activites at the Time of the Occurrence TET Unit 1 was in a long ter cold shutdown ccadition.

II. _Circu= stances Leading to the Occurrence At 0150 on 1/1/83 RM-A9 Iodine channel, which =enitors the Reactor Building purge effluent, failed. A Reactor Evilding purge was in progress at the time.

Shortly bef ore the f ailure a routine source check of the Iodine channel' had been satisfactorily perfor=ed.

III. Descriotion Per action statement 31 of Tech. Spec. Table 3.21-2 the purge which was in regress =ay centinue for up to 28 days, provided that, within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> af ter the channel has been declared inoperable, sanples are continuously collected with auxiliary sa=pling equip =ent. The Reactor Building purge was ter=inated at 0155 hours0.00179 days <br />0.0431 hours <br />2.562831e-4 weeks <br />5.89775e-5 months <br /> on 1/1/83, within the required 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />.

IV. Resultar.t Event No significant occurrence took place as a result cf this event. The particulate and gas channels cf RM-A9 were cperable during the purge. No iodine above background was present since the plant has been in cold shut-down since 1979.

VI. Root Cause Failure of RM-A9 iodine single channel analyzer circuit board.

VII. I==e diate Corrective Actien'

!=nediate ccrrective action was to secure the purge within the required time fra=e. An investigation revealed that the single chuanel analyzer circuit board of RM-A9 iodine channel had f ailed. The circuit board was l- -

LIR 83-01 (Continued) replaced and S? 1302-3.1, PES Quarterly Calibratien, was perfor=ed to essure the channel was functioning properly following repair.

VIII. Lone Term Corrective Action No additional ecErective action is required for this isolated case of conpenent f ailure. .

IX. Cocponent Failure Data Victoreen single channel analyzer circuit board, part number 842-3-6.
