RT INDICATIONS ON SAFETY INJECTION TANKS AND REACTOR PUMP SUPPORTS CONTRACT 3240-02, COMBUSTION ENGINEERING (D/N #023)

In accordance with the provisions of 10CFR50.55(e), Region V was informed of the subject condition on April 28, 1981. Interim Re-ports were subsequently submitted on May 21, July 2 and September 21, 1981.

Attached is an Interim Report on the subject condition. An up-dated or final report will be provided by January 29, 1982.

WASHINGTON PUBLIC POWER SUPPLY 3YSTEM PROJECT NOS. 3 AND 5 DOCKET NUMBERS 50-508 AND 50-509 QUESTIONABLE INDICATIONS ON RADIOGRAPHIC FILM NATURE OF THE PROBLEM The potential problem involves questionable indications on radiographic film of WNP-3 Safety Injection Tanks, Reactor Coolant Pump Volutes and the Reactor Coolant Pump Motcr Supperts. Failure of welos in these components could result in failure of the associated Class I systems to perform their designed safey functions. The components and ques-tionable film were supplied by Combustion Engineering.

Ebasco initiated a Nonconformance Report that identified all radio-graphs found unacceptable due to improper radiographic technique, pro-

~ cess and documentation. Specifically, areas of concern were:

1. Conflict between Ebasco and CE concerning Code requirements for allowable limits of porosity and aggregate slag.
2. Incorrect penetrameter used.
3. Reader sheet incorrect.
4. Geometric unsharpness.
5. Penetrameter not spaced per ASME V qualifying penetrameter cut-off.
6. No penetrameter repair film station markers in weld.
7. Penetrameter and location markers in weld and reader sheet incorrect.

APPROACH TO RESOLUTION OF THE PROBLEM A joint review of the questionable film was performed by Ebasco, CE and the CE vendor. Those issues not resolved or clarified were to be re-solved by taking additional radiographs, performing surface checks and resolving differences in Code interpretation.

STATUS OF PROPOSED RESOLUTION

Based on the evaluation of re-radiographed films and re-examination of questionable surface conditions, CE informed Ebasco that all question-able radiographs (15 total), except two for the safe-end welds on Safety Injection Tanks #3 and #4 of WNP-3, were considered acceptable.

Ebasco,.however, accepted only 10 of the 15 questionable radiographs.

The welds in dispute are the safe-end welds and circumferential welds joining two tank sections of the Safety Injection Tanks.

STATUS OF PROPOSED RESOLUTION (Continued)

Efforts to resolve the two problem areas are as follows:

Safe-End Welds Re-radiography of safe-end welds in Tanks #3 and #4 (WNP-3) found linear indications that were not evident in the orig:nal radiographic film submitted by CE. CE has agreed to repair the welds associated with these indications.

Since the welding process and material configuration of Tanks #3 and

  1. 4 are similar to the remaining six Safety Injection Tanks for WNP-3/5 (four per Unit), Ebasco formally requested CE to re-radiograph welds in the six remaining tanks to determine if a similar condition exists.

Based on a re-review of associated RT film for the WNP-5 tanks, CE contends that a similar condition in the remaining tanks does not exist and re-radiography is not necessary. CE will review radiographs of Tanks #1 and #2 for WNP-3 when they are at site to perform rework on Tanks #3 and #4.

Circumferential Welds Joining Tank Sections CE considers indications found in films of the circumferential welds joining the sections of Safety Injection Tanks #1 and #2 (WNP-3) to be acceptable. Ebasco, however, has identified these indications as rejectable porosity not in accordance with the requirements of ASME Section III, 1974 Edition, Summer 1975 Addendum.

REASON WHY FINAL REPORT WILL BE DELAYED The conflict in Code interpretation concerning rejectable porosity, coordinating weld repair of safe-end linear indications and the deter-mination of whether to re-radiograph safe-end welds of the remaining WNP-3/5 tanks has necessitated further negotiations between Ebasco, CE and the Supply System.

PROJECTED COMPLETION OF CORRECTIVE ACTION AND REPORT It is anticipated that the conflicts discussed above will be resolved by January 15, 1982, and a report identifying resolutions and asso-ciated corrective actions will be subsequently provided by January 29, 1982.
