'On March 11, 1985, the Supply System notified your office of a potential 10CFR50.55(e) deficiency concerning the subject tension wires. As was previously reported in a Supply System Interim Report, dated March 29, 1985, (Letter No. G03-85-167) this deficiency could have adversely affected the safe operation of the plant and is r2 portable in accordance with the provisions of 10CFR50.55(e).

Description of Deficieng Grilles are used to direct air flow exiting from the conditioned air supply ducts to spaces requiring ventilation.

Titus Model 272 Grilles are used in the following safety-related HVAC systems in WNP-3:

Control Room HVAC Electric Equipment Areas HVAC Battery Room HVAC RAB Main Ventilation System Diesel Fuel Oil HVAC

-Diesel Generator HVAC Fuel Handling Building HVAC The Vendor, Titus Products, has identified the following deficiency in their Model 272 Grilles. If the blades are closed, some of the tension wires used for adjustment of blade position will not remain in place.

Titus has stated that this deficiency would prevent the HVAC systems on which the grilles were used from functioning as designed. Consequently, the grille blades would remain shut, thereby preventing air flow. One design objective of these ventilation systems is to supply fresh uncontaminated air to various areas of the plant.

Analysis of-Safety Implications The deficiency, if left uncorrected, could be reasonably postulated to adversely affect the safety of the plant, _i.e., preventing conditioned air flow to critical areas of the plant. This would adversely affect efforts to mitigate or

~ prevent release of off-site radiation. This deficiency, therefore, is considered reportable under the criteria of 10CFR50.55(e).

Corrective Actions Taken The Vendor conducted tests and determined that the reason the tension wires did not remain in place was that, when the blades were fully closed, the tension wires became unhooked and went ' slack and would not bear against the shafts to operate the blades. .The wires also lost contact with the lance which operated the blades.

~ Corrective action involved the replacement of the tension wires and the installation of new push-on locknuts to secure the tension wires to the shafts of the air deflection blades.

.The corrective action has been performed in accordance with Titus procedures and been certified accordingly. The work was performed at the WNP-3 Site, under the supervision of a Titus representative, and is now complete.

No' recurrence of this deficiency is anticipated, since purchase of the grilles

'is complete.
