An investigation is in progress; however, sufficient dat6 has not been received from the Vendor to determine the cause, corrective / preventive actions and safety significance.

Therefore, a Supply System approved Interim Report is attached.

WASHINGTON NUCLEAR PROJECT NO. 3 (DOCKET NO. 50-508 POTENTIAL 10CFR50.55(e) DEFICIENCY INTERIM REPORT DIESEL GENERATOR AIR DEFLECTORS (D/N NO. 59)

DESCRIPTION OF THE DEFICIENCY Louis Allis notified the Nuclear Regulatory Commission of a potential problem invciving possible fractured or cracked welds on the Emergency Standby Generators. The Supply System was identified as an end user of two (2) such generators. These were fabricated by Louis Allis and purchased by the Supply System from Colt Industries as part of the Emergency Standby Diesel Generator Sets.

The welds in question secure the conical air deflectors to the three piece coil guards located at the outboard end of each generator. There are approximately sixty (60) 3/16" fillet welds, one (1) inch long, on three (3) inch centers on each generator. The inspection, as recommended by Louis Allis, consisted of a visual examination for possible cracks or fractures.

Subsequent to this notification, an inspection was performed on the welds in question. The result of the inspection showed evidence of some weld defects (e.g., crater pits, cracked tacks, improper weld termination, improper weld profile).

SAFETY IMPLICATIONS The subject generators supply electrical power to the 4.16KV Class IE ESF Systems in the event of a loss of offsite power. A gross failure of the welds in question could result in the air deflector falling into the internal rotating portion of the generator while it is in service. Such an event, if it were to occur, could result in generator failure. To-date, it has not been determined whether the wela deficiencies could result in weld failure.

CORRECTIVE ACTIONS The noted defects were documented on a Nonconformance Report and transmitted to the Vendor for disposition and determination of corrective action (if any). To-date, a response has not been received. Following receipt of information from the Vendor, appropriate corrective action will be taken and a Final Report will be submitted by September 27, 1985.
