- Hancocks Bridge,.-New-Jersey' 08038-IDENTIFICATION'OF OCCURRENCE:

Radiation (Monitoring Systems ~-- Containment Particulate-4 Monitor -' Inoperable.-

This.reportiwas initiated by Incident Report'82-506.

' CONDITIONS' PRIOR TO OCCURRENCE:,

Mode 1

'Rx Power 82% - Unit - Load' 9 20 MWe.

DESCRIPTION OF OCCURRENCE:

At'1501 hours0.0174 days
0.417 hours
0.00248 weeks
5.711305e-4 months
, December 6,- 1982, during routine operation, the-Control Room Operator discovered-that the containment particulate-

~

radioactivity monitor. (Channel 2RllA) had malfunctioned.

The-channel' digital display indicated it was electronically locked Eup.

The instrument was declared inoperable and Limiting Condition for Operation 3.3.3.1 Action 24 was entered. 'The redundant Reactor Coolant System (RCS) leak detection systems, including the Containment Sump Monitoring System and the containment gaseous activity and fan cooler condensate flow rate instruments, were operable throughout the occurrence.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The channel was powered down then re-energized to. reset the-logic; instrument response and power supply voltages checked satisfactorily.

Major integrated circuits and circuit boards were reseated.

The channel tested satisfactorily and was returned to operation.. No further problems have been noted, and the occurrence was assumed.to involve an intermittent connection on one of the components which were reseated.

ANALYSIS OF OCCURRENCE:

Operability of the containment particulate monitor insures that the levels of radioactivity in the containment atmosphere are continually measured. 'As noted, the redundant RCS leak detection

[

systems were operable and radioactivity levels in the containment were continuously monitored by the containment gaseous monitor.

i The event therefore involved no risk to the health and safety of i

the public.

' ANALYSIS OF OCCURRENCE:

.(cont'd)

TLimiting-Condition for.-Operation 3.3.3.1' Action 24. requires:.

With"the number'of channelsfoperable less than~the minimum number of channels operable required,. operation may. continue for up-to' thirty days, provided that grab samples of the containment' atmosphere.are obtained, and analyzed at.least once per.24 hours2.777778e-4 days
0.00667 hours
3.968254e-5 weeks
9.132e-6 months
; otherwise be in at least hot; standby within the next 6 hours6.944444e-5 days
0.00167 hours
9.920635e-6 weeks
2.283e-6 months
and in cold shutdown within the following 30 hours3.472222e-4 days
0.00833 hours
4.960317e-5 weeks
1.1415e-5 months
.

CO'RRECTIVE ACTION:

Following satisfactory testing, Channel 2RllA was -declared operable, and at'2139 hours0.0248 days
0.594 hours
0.00354 weeks
8.138895e-4 months
, December 6, 1982, Limiting Condition for Operation 3.3.3.1 Action 24 was terminated.

As stated, since reseating of the integrated circuits and boards, no-further problems were noted.

Intermittent connections in digital electronics circuitry on an isolated basis are not unusual.

In view of-the nature of.the occurrence, no furtner action was' deemed necessary.

FAILURE DATA:

A previous similar occurrence involving the containment gaseous activity monitor was documented.in LER 82-133/03L.
