Upo' ate to Response to Inspection Report 50-285/89-29 Reference 2 identified several emergency exercise weaknesses to which OPPD +

responded in Reference 3. One of these weaknesses was Item 8929 05 which stated:

The fact that timely personnel accountability during evacuation of the $

protected area was not demonstrated is considered to be a weakness.

OPPD's " Actions to Prevent Recurrence" included the following statements:  ;

L A timed accountability drill or drills will be tested before December 31, 1989 to demonstrate and verify the improved method. A letter i explaining the new evacuation accountability method will be issued to  :

all badged personnel by February 28, 1990. The new method will also be incorporated into General Employee Training (GET).

Proposed revisions to the station's accountability and evacuation procedures were prepared in September, 1989 and subsequent training was completed. During the month of December,1989, several drills were initiated to evaluate the improvements. Additional weaknesses, including training weaknesses within the t Security Department were noted. Thorough walk-throughs were conducted with each Security Department shift. Final training for personnel was completed, l-and the applicable security accountability procedures were issued on January 18, 1990. On January 31, 1990, during implementation of a planned pre-outage emergency drill, the new process for evacuation and accountability was demonstrated and successfully completed within 30 minutes.
