.o A esuG:racce mieynumu my Sequoyah Nuclear Plant LER SUPPLEMENTAL INFORMATION SQRO-50-327/80180 Technical Specifications Involved: 3.0.3, 3.5.2, and 3.6.2.1 Reported Under Technical Specification 6.9.1.12.1 Date of Occurrence: 11/14/80 Time of Occurrence: 1000 CST Conditions Prior to Occurrence Mode 3, RCS Temperature 547 degrees F and RCS Pressure 2235 psig Identification and Description of Occurrence In response to IE-Bulletin 80-11, Engineering Design evaluated the ability of certain non-reinforced concrete masonry block walls to withstand a seismic event and determined that some walls could fail. The failure of each of the walls was evaluated to determine if safety-related equipment would be affected.

At 1000 CST on 11/4/80, the plant was notified that both containment spray pumps could be jeopardized by a failure of the block walls of.the respective pump rooms.-

The pumps were declared ineperable and the plant immediately initiated action ,

to go ' to mode 4 as requires. by LCO 3.0.3.

At 1630 CST o:. 11/14/80 the plant was notified that both RHR pumps could be

' jeopardized by the containment spray pump 1B & 2B room walls and that centrifugal charging pump 1B could be jeopardized by centrifugal charging pump 1B & 2B room walls.

At 0503 CST on 11/15/80 plant entered mode 5.

Apparent Cause of Occurrence The non-reinforced concrete block walls were not initially designed for a seismic event.

Corrective Action Restraints were designed and-installed for the walls affecting unit 1 safety related equipment as shown below:

ROOM RELATED EQUIPMENT Cent. Chg. Pump 1B (inside)-- CCP 1B Jent. Chg. Pump 2B (outside) CCP 1B CCS Pump'lA (both sides) CCS LA CCS Pump 13 (both sides) CCS 1B, RHR LA CCS Pump 2 B (outside) RHR 1B Restraints will be installed _ to protect unit 2, equipment prior to startup of that unit.
