On November 4, 1989 during the course of performing a portion of the drywell Integrated Leak Rate Test (ILRT) procedure, a full Reactor Protection System (RPS) signal was inadvertently generated while pressurizing the drywell. This resulted in a full scram signal (no control rod motion), with the only plant response being annunciation and closure of the scram discharge volume vents and drains. A review of the ILRT procedure determined that a set of contacts in the RPS logic which needed to be jumpered to prevent such an occurrence was not included in the procedure. As such, the root cause of this incident was classified as o procedural deficiency. Corrective actions consisted of the Inservice l Inspection (ISI) group revising the ILRT procedure to include the necessary RPS contacts.

IDENTIFICATION OF OCCURRENCE Full Reactor Protection System (RPS) Initiation During Integrated Leak Rate Testing Due To Procedure beficiency Event Date: 11/4/89 Event Time: 1734 This LER was initiated by Incident Report No.89-156 CONDITIONS PRIOP TO OCCURRENCE
-Plant in OPERATIONAL CONDITION 5 (Refueling), ILRT in progress.
DESCRIPTION OF OCCURRENCE On November 4, 1989 during the course of performing a portion of the drywell ILRT procedure, a full RPS signal was generated during pressurization of the drywell. Per design, the signal was generated at 1,68 PSIG. -This action resulted in a full scram signal. (no control rod motion), with the only plant response being annunciation and closure of the scram discharge volume vents and drains. After verifying the cause of the scram signal, control room personnel reset the scram signals and drained the scram discharge volume. A four hour non-emergency report was initiated IAW 10CFR.50.72, and an l incident report was written to document the event.
APPARENT CAUSE OF OCCURRENCE The- root cause of this occurrence was classified .as a i
procedural deficiency. A review of the ILRT procedure determined that a set of contacts in the RPS logic which needed i to be jumpered to prevent such an occurrence were not included l in the procedure.
L L EBEVIOUS OCCURRENCES There have been no similar occurrences when performing the ILRT
, procedure, as this was the first time the subject procedure had L been performed. The ILRT procedure is performed only once i every four years, and had not been performed since plant l construction in January, 1986 as a Prooperational Test Procedure I (PTP).

0 0 03 m' 0 3 SAFETY SIGNIFICANCE This incident presented minimal safety significance, other than an unnecessary challenge to the RPS logic. All other actuations and initiations expected of a high drywell pressure signal had been appropriately jumpered prior to conducting the ILRT.
CORRECTIVE ACTIONS The Inservice Inspection group revised the ILRT procedure to include the RPS contacts which were not originally jumpered.
This corrective action will preclude a repeat of an event similar to this the next time the ILRT procedure is performed.
