On August 26, 1992, the time allowed per unit for condenser chlorination exceeded the limit allowed by the National Pollutant Discharge Elimination System (NPDES) Permit for St. Lucie Unit 1.

This deviation is being reported to the Environmental Protection Agency in the Monthly Discharge Monitoring Report. A copy of the event is attached and is being sent pursuant to Section 3.2.1 of the St. Lucie Unit 1 Environmental Protection Plan.

ATTACHMENT On August 26, 1992, the time allowed per unit for condenser chlorination was exceeded at the St. Lucie Plant. Unit 1 was chlorinated for 110 minutes in the morning. That afternoon, when Unit 2 was scheduled for chlorination, Unit 1 was mistakenly chlorinated again for 110 minutes. This exceeds the permitted time of 120 minutes per unit per day. In that the total chlorine to the discharge canal was no more than normal, no environmental damage was done.

The root cause was human error. It was further complicated by poorly designed daily logs that document chlorination operations.

To prevent urther occurrences, the Operations Department has done the following:

1) Operating Procedure 0630022, "Hypochlorite System Operation",

has been revised to include a sign-off verification that the unit to receive chlorination has not been chlorinated that day.

2) An operations night order has been issued, reiterating that the operator performing hypochlorination will notify the Control Room Reactor Operator prior to beginning the evolution.
3) The Control Room Chlorination Log has been revised to more clearly reflect the unit being chlorinated and previous daily history.
