Dear Mr. Powell:

As a follow-up to our July 14, 1992 verbal report to the FDER, this written report is being provided for the subject event as you requested. At approximately 2:30 a.m. on July 14, 1992, St. Lucie plant personnel discovered a loss of pump suction on the Unit 2 hydrazine chemical feed system tanks. An investigation revealed that the chemical feed tanks, which collectively hold approximately 200 gallons of dilute hydrazine solution, were empty. Further investigation revealed that a drain valve from a pump connected to the tanks was partially open. An analysis of activities leading up to the open drain valve discovery revealed that the probable cause was inadvertent bumping of the drain valve to the open position during maintenance on the chemical feed system on July 13, 1992.

The released hydrazine solution resulted in the discharge of approximately 72 pounds of hydrazine to the storm drain system which drains to the onsite West Stormwater Basin, an evaporation/percolation basin with no surface water outfall. FPL does not believe that the release constituted an off-site release as defined under Section 304 of SARA Title III, but was reportable under CERCLA, since hydrazine has an RQ of one (1) pound. The event was reported to the National Response Center and as a courtesy to the State Emergency Response Commission and the District 10, Local Emergency Planning Committee on the morning of July 14, 1992.

The release did not result in any on or offsite health or safety impacts and no emergency response and/or clean-up actions were required. Plant personnel are investigating corrective action measures. Initially, the drains from all chemical injection pumps have been capped to prevent releases to the storm drain system. Additionally, the engineering feasibility of secondary containment for the chemical feed system is being investigated.
