This submittal meets the requirements in 10 CFR 26.203, 10 CFR 26.717 and 10 CFR 26.719.

In accordance with 10 CFR 26.11 copies of the reported information are also enclosed with this report to the appropriate regional office and resident inspector.

Topic I Topic 1 Description Wolf Creek continues to review and update it's Fitness For Duty Procedures and Policies in an effort to Policies and Procedures continue its compliance with NRC rules and regulations. Wolf Creek reverted back to split collections in October 2011. The split process is consistent with Department of Transportation collections and will strengthen the Appeal process when a re-test is requested by an individual for confirmation at a second laboratory.

NRC FFD Program Performance Data Reporting System 10 CFR Part 26, Subpart I - Managing Fatigue Attachment This attachment provides the entire narrative as the NRC form has been created with boxes that scroll down, and when printed, the information does not print in its entirety due to the way the NRC has built the electronic FM form.

Analysis of Waiver Assessment Data:

No waivers were initiated at Wolf Creek for 2011.

Analysis of Fatigue Assessment Data:

Fourteen Fatigue Assessments were completed from 01-01-2011 through 12-31-2011. Seven were for-cause assessments and seven were post event assessments. Wolf Creek had zero fatigue assessments completed for a waiver or for a self-declare of fatigue. Two workers did self-declare during the 2011 planned refueling outage and were sent home for a minimum of a 10-hour break and no fatigue assessment was performed or required.

Conclusions:==

Wolf Creek initiated zero waivers for 2011. For the post-event and for-cause assessments, adequate actions were taken in sending the workers home pending fitness-for-duty results. In review of the completed fatigue assessments no program or performance issues were noted from a collective review of the data.

Summary and Status of Corrective Actions:

The evaluation review period is 01-01-2011 through 12-31-2011. During this review period no waivers were performed, fourteen fatigue assessments were performed, and two workers self-declared fatigue.

Each fatigue assessment and both self-declares were documented in the corrective action program. In review of the condition reports generated for fatigue assessments and self-declares, the corrective actions taken supported fatigue rule requirements. In a collective review of the 2011 fatigue assessments and self-declare data no gaps to performance or program improvement opportunities were found.
