NRG FFD Program Performance Data Reporting System 10 CFR Part 26, Subpart I - Managing Fatigue Attachment This attachment provides the entire narrative as the NRC form 892 has been created with boxes that scroll down, and when printed, the information prints very small due to the way the NRG has built the electronic FM form.
Analysis of Waiver Assessment Data:
No waivers were issued for 2019.
Analysis of Fatigue Assessment Data:
Eight (8) Fatigue Assessments were completed in 2019.
2 Fatigue Assessments/Post-Event, 1 Fatigue Assessment/For Cause.
5 Fatigue Assessments/Self-Declaration No additional actions were taken beyond worker being sent home for minimum of ten hours pending FFD test results. One recommendation made from the Fatigue Management Program Review was to evaluate if improvements could be made in the process for self-declare. Reference CR 00140181

Conclusions:==
For 2019 there were four (4) violations of work hours, zero (0) waivers, and eight (8) fatigue assessments were performed. One recommendation condition report was generated from Fatigue Management Program Review to evaluate and determine if process improvements can be made for self-declaration of fatigue.
Reference CR 00140181 Performance improvement is noted for RF23 in contractors tracking of time and not incurring work hour violations during the refueling outage. Ownership and accountability are evident as this performance is outstanding and acknowledged by the site.
Summary and Status of Corrective Actions:
The Fatigue Management Annual Program Review assessment did not document any performance gaps or additional issues. Reference QH-2020-1977.
