This submittal meets the requirements in 10 CFR 26.203, 10 CFR 26.717 and 10 CFR 26.719. In accordance with 10 CFR 26.11, copies of the reported information are also enclosed with this report for the appropriate regional office and Senior Resident Inspector.

Topic 1 Topic 1 Description Random Testing In 2021 there have been 5 condition reports initiated for individuals arriving late for their scheduled FFD random test. CR 10006150 was written to perform a collective review of these issues to determine if any actions could improve performance or the process. Reference condition reports 00146543, 10000691, 10002986, 10003105 and 10006114.

Actions taken for CR 10006150: Those missing future tests will have a face-to-face discussion with their Supervisor and the Plant Manager or his designee to explain the reason for the missed test and actions that will be taken to prevent this in the future. Discussed this expectation with Access Screening and the Plant Manager.

Topic 2 Topic 2 Description Program and System Management During the NRC Fitness for Duty Inspection 71130.08 it was identified the FFD refrigerator used to store specimen collections was reading between fluctuating between 43-and 44-degrees Fahrenheit.

10 CFR 26.117(j) states: Specimens that have not been shipped to the HHS-certified laboratory or the licensee testing facility within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> of collection and any specimen that is suspected of having been substituted, adulterated, or tampered with in any way must be maintained cooled to not more than 6°C (42.8 °F) until they are shipped to the HHS-certified laboratory.

The temperature was immediately corrected by adjusting the temperature set-point, high limit alarm and low limit alarm to the appropriate settings. Previous setpoints that were set on the fridge during installation were 42.1 and 62. A documentation audit was performed on specimens collected since new fridge was installed on 11-23-2020 which proved all specimen collections were able to be tested with no issues. Additional Validation: One of our Medical Review Officers provided validation that the 2-degree Add an additional Topic temperature difference had no effect on the specimens. Actions taken: The manager of maintenance is to provide training on this issue and give clear and specific guidance on regulatory requirement to not violate NRC work hour rules. Due date is 4/28/2022. (OPEN) 10 Topic 2 - (Historical) The 2021 NRC fitness for duty inspection exit meeting, the lead inspector indicated he would be documenting in the inspection report a licensee-identified, Green non-cited violation (NCV) of 10 CFR 26.31(c)(3) which requires that as soon as practical

after an event involving a human error, where the human error may have caused or contributed to the event, the licensee or other entity shall test the individual(s) who committed the error(s). Contrary to this, QA audit 18-09-FFD/FM identified that Fatigue Assessments were 11 - 20 not performed on 4/12/18 and 6/21/18. Licensee-identified NCVs are not assigned cross-cutting aspects. CR 127487 was written for the original issue identified by QA. CR 10001396 was written to track this violation. (CLOSED)

Topic 3 - During RF24, Maintenance scaffold leads were loaned to Engineeri ng Proj ects to support scaffold activities. Projects leadership had the responsibility for assigning outage schedules and admin codes to apply outage rules and approve timesheets in EmpCenter. It was recognized that 2 Maintenance Support employees that were on-loan to Projects during RF24 were assigned daily schedules at the end of RF24; however, they were left on outage rules. Outage rules are not as conservative as on-line rules and are only to be applied when we are off-line. The appropriate Admin

Code was applied, and the timesheets were re-approved, which allowed verification that no work hour violations occurred during this time.

Actions taken: CR 10008832 (CLOSED) Event shared with all work hour tracking timekeepers to raise awareness of the importance in task completion and verification for loaned individuals.

General Comments:

The Fatigue Management Program Review did not document any performance gaps or additional issues.
