LIC-04-0081 Attachment Page 3 I. MANAGEMENT ACTIONS A. One non-licensed employee tested positive for alcohol on a random test. He then tested positive for alcohol again on a re-entry test prior to completing a re-entry agreement.

B. A contractor employee attempted to subvert his drug/alcohol test by providing urine outside the temperature range. He was then directly observed while providing a second specimen. The second (observed) specimen tested positive for opiates.
