On December 31, 2022, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Palo Verde Nuclear Generating Station. On January 6, 2023, the NRC inspectors discussed the results of this inspection with you and other members of your staff. The results of this inspection are documented in the enclosed report.
No findings or violations of more than minor significance were identified during this inspection.

SUMMARY
The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Palo Verde Nuclear Generating Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS
Units 1 and 2 operated at or near full power for the duration of the inspection period.
Unit 3 entered the inspection period near full power during a coastdown for refueling outage
3R23. On October 7, 2022, the unit was shut down for the refueling outage. On November 9,
2022, the unit reached criticality. During power ascension, the licensee identified a power supply issue in the turbine electrohydraulic control system and lowered power from 68 to 12 percent power to replace the power swap over relay on November 15, 2022. On November 16, 2022, the licensee began power ascension and returned to full power on November 18, 2022, where it remained at or near full power for the remainder of the inspection period.
INSPECTION SCOPES
Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed on-site portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

Problem Identification and Resolution.
The inspectors reviewed 11 condition reports associated with inservice inspection activities. No findings or violations of more than minor significance were identified.

- (1) The inspectors reviewed and evaluated the licensed operator examination failure rates for the requalification annual operating exam administered on August 17 to September 30, 2022.

- (1) Biennial Requalification Written Examinations The inspectors evaluated the quality of the licensed operator biennial requalification written examination administered on August 17 to September 30, 2022.
Annual Requalification Operating Tests The inspectors evaluated the adequacy of the facility licensees annual requalification operating test.
Administration of an Annual Requalification Operating Test The inspectors evaluated the effectiveness of the facility licensee in administering requalification operating tests required by 10 CFR 55.59(a)(2) and that the facility licensee is effectively evaluating their licensed operators for mastery of training objectives.
Requalification Examination Security The inspectors evaluated the ability of the facility licensee to safeguard examination material, such that the examination is not compromised.
Remedial Training and Re-examinations The inspectors evaluated the effectiveness of remedial training conducted by the licensee, and reviewed the adequacy of re-examinations for licensed operators who did not pass a required requalification examination.
Operator License Conditions The inspectors evaluated the licensees program for ensuring that licensed operators meet the conditions of their licenses.
Control Room Simulator The inspectors evaluated the adequacy of the facility licensees control room simulator in modeling the actual plant, and for meeting the requirements contained in 10 CFR 55.46. The inspectors reviewed a rod control operating system modification to simulator A, and the associated crew training and testing plan.
Problem Identification and Resolution The inspectors evaluated the licensees ability to identify and resolve problems associated with licensed operator performance.

71152S - Semiannual Trend Problem Identification and Resolution Semiannual Trend Review (Section 03.02)
- (1) The inspectors reviewed the licensees corrective action program for potential adverse trends that might be indicative of a more significant safety issue. The inspectors performed an in-depth review of the licensees ability to identify issues regarding radiation monitors, evaluate each occurrence, and implement corrective actions. During this review, the inspectors noted an increasing trend in the number of condition reports written on radiation monitors and the radiation monitor communication in all three units. The licensee was aware of the declining performance of the radiation monitoring system, having placed the system in maintenance rule (a)(1) status in 2018 with corrective actions in place to modernize the system due to its age and limited available replacement parts resulting from the original vendor going out of business. The inspectors determined that the licensee has adequately followed site procedures to evaluate and correct the identified issues with the radiation monitor system and continue to monitor system performance while executing the established corrective actions. No more than minor concerns were identified.
