Unannounced inspection by a regional inspector and a

contractor of the fire protection program and systems at Palo Verde Nuclear Generating Station.

The inspection involved 64 hours7.407407e-4 days <br />0.0178 hours <br />1.058201e-4 weeks <br />2.4352e-5 months <br /> onsite each for the NRC and contract inspectors.

The contractor inspection summary has not yet become available and any additional results will be included in Inspection Report 50-528/84-62 which will be conducted prior to licensing.

Results:

Of the areas examined no violations or deviations were identified.

Two unresolved items (See paragraph 4) were identified.

Procedures The licensee administrative procedures were reviewed to ensure that they met the commitments of the FSAR and Branch Technical Position 9.5.1.

They appear adequate to meet licensee commitments.

The licensee surveillance procedures were reviewed to ensure that the requirements specified in the proposed technical specifications are met.

They appear adequate to ensure compliance with their technical specifications.

Fire Bri ade Trainin The licensee had committed to perform the training specified in

CFR 50, Appendix "R" regarding the fire brigade.

The content of the training was compared to the Appendix "R" training requirements and course content satisfactorily met the requirements.

The list of qualified fire brigade members provided by the licensee's training section had no provision to delete the names of personnel who for some reason was no longer on the fire brigade.

The inspector had to use the lists of names provided by the cognizant supervisors.

From these lists the training records for a randomly selected group of fire brigade members were examined.

The training appeared to meet the requirements of Appendix "R" to 10 CFR 50 and all members examined appeared to be current. in the periodic training and drills specified by 10 CFR 50, Appendix "R".

SER Follow-u The Safety Evaluation Report (SER) stated that the licensee had committed that a single line break in a water supply would not eliminate both primary and secondary water supplies.

In one area of the Auxiliary building it is not clear that this is the case.

The licensee has a

procedure to use a fire hose to cross connect the systems in the event of a line break, to ensure that both systems are not eliminated.

It was not clear that the SER evaluated this method.

The licensee is in the process of resolving this issue with NRR.

This item is unresolved.

The cable spreading rooms have large heavy metal missile doors for their entrance.

The SER did not evaluate the use of these doo'rs as fire barriers and they are not rated as fire doors.

The licensee i's resolving this issue with NRR.

05o The licensee had committed to install, in the SER in 1981, additional fire detectors, portable extinguishers, and-automatic sprinklers in areas not specified in the SER (PJ-82-'05).

In the four years since the SER was written, many additional detectors,

'extinguishers and sprinklers were added throughout the plant.

At the time of the inspection the inspectors found only one area where an additional fire extinguisher was needed (in or near fire zone 37C and 37D) and no additional detectors or sprinklers needed.

The licensee corpnitted to add a fire extinguisher in the specified fire zone (50-528/84-Q-03)

(Open) Item PJ-82-05 is closed.

Fire Protection Pro ram The licensee had committed to have their plant fire protection administrative controls and procedures implemented prior to fuel load (PJ-82-06).

At the time of the inspection this program had not been implemented.

The licensee has committed to have this program implemented on December 1,

1984.

The inspector will follow-up at a later inspection prior to fuel load (PJ-82-06)

Charcoal Filter Su ression S stem In the SER the licensee committed to install a fixed pipe water suppression system to the containment charcoal filters't the time of the inspection the pipe connections had not yet been installed.

The inspector will verify at his inspection prior to fuel load that the pipe connection has been installed (50-528/84-Q-04)

tube Oil Collection S stem The licensee agreed to install a lube oil collection system for the reactor coolant pumps in accordance with Section III.O of 10 CFR 50, Appendix "R".

Other I,icensee Commitments The inspector verified by direct observation that the Remote Shutdown Panel T-Cold indication and the fire detectors which the licensee had committed to install in a letter, dated October 16, 1984, to Mr. Knighton of the NRC, were installed.

Penetration Seals The applicant provided documentation to show that the 'fire rated penetration seals used in the unit had passed a 3-hour fire test in accordance with the ASTM E-119 fire test procedure.

Valve Su ervision All major water supply or isolation valves in the fire protection water system are either electrically supervised or key-locked open as committed by the licensee.

However, at the time of the inspection, the systems were still being tested and the electrical supervision and locks were not being used in every case.

The inspector will verify thap the system is in use at his inspection prior to fuel load (50-528/84-P'9-05)

Fire Protection E ui ment The fire brigade turnout gear, breathing apparatus and emergency response equipment appeared to be in good condition.

The in-plant fire suppression equipment such as fire hoses, extinguishers and sprinkler systems seemed to be well maintained and in good condition.

The inspector found no obstructions around the sprinklers or other fire suppression equipment and the location and availability of the equipment seem adequate.

I Other Concerns 13.

There are several locked metal enclosed temporary contractor work areas inside of the cable spreading rooms.

The inspector was concerned that licensee inspection personnel would not be able to ensure proper material storage inside of these enclosures in the vital areas.

The licensee committed to change his inspection procedure to inyure that these enclosures are periodically inspected (50-528/84-P-06)

In the event of an emergency, personnel in the plant are required to notify the control room by using the telephone system.

There are currently no permanent lists of emergency phone numbers installed at the telephone locations.

The licensee committed to put stickers listing the emergency telephone numbers at each phone location (84-Jr9-07)
