Statement of Vi,clation Failure To Follow The Approved Procedure For Performing Maintenance

l South Texas Project (STP) Technical Specifications (TS), Section 6.0 l
(Administrative Controls), Specification 6.8.1.a requires that written
)
procedures shall be established, implemented, and maintained for the
{
control of maintenance and repair activities.
Plant Procedure OPGP03-ZM-0003 implements the requirements of this TS including a
provision.110 wing emergency repairs without the use of an approved l
Maintenance Work Request (MVR).

Contrary to the above, a repair activity was observed on the fuel i
handling system transfer cart shear pin in violation of the procedure.
l An MWR had not been approved and the repair work was improperly commenced on an emergency basis when there was no direct safety hazard to personnel j

or equipment which would justify an emergency classification.

II. Reason for Violation j
I The root cause for this violation was a lack of understanding on the part
)
of the shift supervisors of what constitutes an emergency situation and I
when an Emergency MWR could be used.
III. Corrective Action Taken and Results Achieved At the time of discovery of this situation an appropriate MWR war l
initiated to document and correct the problem.
j IV. Corrective Steps Taken to Prevent Recurrence By procedure, only shift supervisors can authorize an Emergency MWR.
j Accordingly, all shift supervisors have been instructed by plant l
management as to intent and proper use of an Emergency MWR.
In addition, i
the MWR procedure has been revised to add a more explicit definition of when an Emergency MWR can be initiated.

STP is in full compliance at this time.
